      • -Choking hazards were observed in the Lavender Room. They were removed at the time of inspection.

      • -Baseboards were peeling from the wall in the Basil Room. Operator had already purchased supplies to repair the wall.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • -Chlorine spray bottles in the washroom were stored on the counter, accessible to children. Please have a designated area to store spray bottles out of children’s reach.-There was a full mop bucket in the washroom. Please empty mop water after use. Ideally mop buckets should be stored in an area not accessible to children.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • -Please install corner protectors on the low white shelf in the Lavendar Room as the corners are sharp-The area rug in the Lavendar Room was rolling up at one corner creating a potential tripping hazard. Please address.
    • 06. Are proper protocols being followed in regards to client care?
      • -Staff were observed diapering on the floor in the Mint Room. Please ensure the mat is cleaned and disinfected on both sides before returning it to the counter.-The diaper mat was stored on the floor of the Jade Room washroom. Please ensure there is a designated storage spot outside of children’s reach.
    • 08. Is this facility properly constructed (Non-Critical)?
      • -The accessible washroom had water stained ceiling tiles. Please replace.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • - The storage room at the back of the facility was very cluttered. This makes it hard to clean and could lead to the attraction of pests. Please clean.-Tape had been used to cover a toilet in the Jade Room and when there was remaining residue. Please clean.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • -There was a climber in the OSC Room but no padding. Please remove or ensure protective padding is available.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • -The wicker floor seats in the OSC were coming apart. Please repair/remove.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • -In the Lavendar Room cots were stacked so that sheets were touching. Please address.

      • -There were multiple colourful foam shapes with bite marks and missing pieces. This makes the shapes difficult to clean and is a potential choking hazard. The damaged shapes were removed at the time of inspection.
