Ensure that all semi-critical tools are disinfected using minimum high-level disinfectant.2- High level disinfectant not being used according to manufacturer instructions for use.3- High level disinfectant was not tested daily prior to reuse.4-No high-level disinfectant testing record was maintained in the facility for review during inspection by EPH.Ensure the high-level disinfectant is used according to manufacturer's instructions for use.HLD must be tested every prior to reuse and record must be maintained.

      • Dish washing and handwashing were being done in the reprocessing area. Designated reprocessing area must be used only for reprocessing of semi-critical tools.Hand washing should not be done in the reprocessing sink.
