    • 08. Is this facility properly constructed (Non-Critical)?
      • Diapering change pad in the infant room was cut to size leaving an exposed edge repaired with tape and staples.Remove diapering pad
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Toy bins in BASC room had visible debris in the bottom. When cleaning and disinfecting toys, the bins must be cleaned and disinfected.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • Water temperature measured in the basement classrooms reached a maximum of 62.2C. Reduce hot water temperature to a maximum of 54C (Recommended temperature of 49C)

    • 08. Is this facility properly constructed (Non-Critical)?
      • 1. Water damage was noted on the bathroom ceiling in the preschool room. Toilet paper was also noted on the ceiling to prevent the dripping water.**Remove the toilet paper from the ceiling. Remove water damaged ceiling tiles. Ensure the roof is repaired to prevent further leaks. Replace with new materials.**2. Water damage was noted on the ceiling of the bathroom in the BASC room. **Remove water damaged ceiling tiles. Ensure there is no longer a leak. Replace with new material.**3. There was a crack on the window in the infant room from the hail storm. Waiting on insurance and repair company to come replace the window.
    • 16. Does the social care facility have adequate lighting?
      • The lighting in the infant bathroom was very dim. **Repair or add an additional light in the bathroom so that there is adequate lighting for changing diapers and handwashing.**

    • 10. Is a properly supplied first aid kit provided and readily available?
      • BASC room: No antiseptic wipes in backpack first aid kit. Replenish anti-septic wipes.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • ECS room: Client chairs had significant buildup of grease and dirt. Clean chairs with soap and water, scrub in ridges of chair, rinse, and disinfect with 100 ppm bleach. Tables are worn, repair or replace.Vents throughout the facility have buildup. Have the ducts cleaned and provide supporting documentation.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Thorough cleaning of the entire facility is needed including all hard-to-reach areas as build up is evident.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Ensure that the mats have more distance between each one during nap time.Ensure that the plastic bags that hold mats are placed away and not on top of toys for children to play with as this poses a serious risk to the children.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • Storage areas in classroom are in disarray. Do not use cardboard boxes for storage.Remove items not in use.
    • 28. Are washrooms / bathing facilities properly constructed, maintained and supplied (Critical)?
      • Toilet in the toddler room is covered with a black garbage bag, when asked why, informed that the toilet is not working. Repair the toilet.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Ensure that the exit door in the toddlers room closes/ opens properly as there is resistance and the door does not close properly.
