- Observation(s): The system to ensure that medication administration records are completed is ineffective. A review of Medication Administrative Record's (MAR) identified PRN medication that was sporadically missing signatures for the administration of medication. Failure to chart or administer medication as prescribed can potentially affect the health and safety of persons in care. Submit by January 9, 2026, the plan that will be implemented to ensure ongoing compliance with Section 78(2)(b) of the Residential Care Regulation. The plan must include your process for ongoing monitoring of the updated system.

- Observation(s): The system to ensure that administered medications are recorded is ineffective. A review of the Medication Administration Records identified several PRN medications which were administered but not recorded showing the date and effectiveness. There were also noted missing signatures for PRN’s that had been recorded with dates and effectiveness.

- Observation(s): The system for self-monitoring the facility is ineffective as noted by the following deficiencies that were observed; - An unlocked door, which had a sign saying keep locked. - A review of daily checklists identified missed recordings - Behavioural support plans located in the red binder, resident care plan binder and on Sharevision had three different review dates for the same plan. - A review of a health care plan in a binder had a review date of January 2023, which was not updated to reflect the current care plan. - A policy and procedure manual accessible to staff was dated April 2023, which was not updated to reflect the current policies and procedures located on Sharevision.

- Observation(s): The system to ensure food is safely stored is ineffective, raw vegetables were observed to be stored openly on a top shelf in a fridge.

- R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
- Observation(s): The system to ensure that all rooms and common areas are maintained in a safe condition is ineffective as evident by the downstairs patio cluttered with furniture and cardboard boxes. The upstairs patio had a pile of extension cords outside the door posing a potential unsafe tripping hazard. The system to ensure that staff are not smoking on the premises is ineffective. A table and chairs with an ashtray containing cigarette butts were observed on an outside patio next to a side door. The system to ensure all medications are safely and securely stored is ineffective. Medicated cream was located in a person in care’s bathroom during this inspection. This was corrected during the inspection.

- Observation(s): The system to ensure that only medication prescribed or ordered by a medical or nurse practitioner is ineffective. It was observed during the inspection that over the counter, non-prescribed medication was stored in the medication cupboard as well in the PRN cupboard.

- Observation(s): The system to ensure that monitoring the health and safety of each person in care regularly to determine if their needs continue to be met is ineffective. A review of a care plan identified that a follow-up medical appointment had not been made.

- Observation(s): Licensing reviewed employee records. There is no system in place to to obtain evidence employees have complied with the province's immunization control program.

- Observation(s): Licensing reviewed the facility policy for outbreak prevention and control. The facility policy provides direction for outbreak control, however, does not provide direction for outbreak prevention.

- Observation(s): The process for monitoring the weight of each person in care monthly is ineffective. Upon review of a person in care's file, several months in 2020 were not documented. There is no process in place to document when a person in care refuses to be weighed.

- Observation(s): The process in place for ensuring that food is safely stored is ineffective. During the inspection, it was noted that a deep freeze and a mini fridge were not monitored to ensure food was safely stored. Also, a fridge was noted to be consistently recorded to have a temperature of -5 degrees Celsius; it was determined that the thermometer was inaccurate. There was also no directions instructing staff on what to do if the temperature readings are not within the describe ranges.
