      • There were no paper towels near the handwashing sink.The operator kept paper towels near the handwashing sink upon PHI's request.

      • Sterility assurance documentation was not maintained for the microneedles used.

      • Wooden foot filers were being re-used.The operator was advised that wooden foot filers are porous and not cleanable; therefore, could not be re-used. The operator removed the wooden foot filers from the service area.

      • A rusted nail clipper was stored with clean tools.The operator removed the rusted nail clipper from the service area.

      • 1) A high-level disinfectant was not available in the facility to disinfect semi-critical tools.The operator informed that Pre-empt RTU (an intermediate-level disinfectant) was used to disinfect semi-critical tools.The operator was advised to stop the manicure/pedicure services until a high-level disinfectant was acquired.2) There was a large metal foot filer which could not be fully immersed in the box used for disinfecting tools.The operator was advised to acquire a box that could fully immerse the large equipment for proper disinfection.
