      • Several lights were burnt out in the main service area. A contractor is scheduled to come in to replace the lights.

      • Staff were unaware if there were written procedures were available for skin cleansing or reprocessing sink decontamination.       • There were hair and debris in the tray of biomers disinfectant that was being used to disinfect esthetic tools. Tools are not being cleaned properly prior to disinfection.

      • Text message from Jasmine with a photo of the HLD5 high level disinfectant purchased. The corresponding test strips for verifying the high-level disinfectant concentration must be available for testing each day of use and recording the results.       • Several service chairs were in disrepair and covered in duct tape. The chairs were no longer cleanable. Refinish or replace.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • The hand sink in the main service area is used for both hand washing and reprocessing equipment without a decontamination step in between. The sink must be decontaminated in between uses.
