      • The cubbies in the toddler and preschool room were not secured to the wall. Secure the cubbies to the wall/floor to prevent them from tipping.

      • i. The rubber along the bottom of one of the stools in the preschool bathroom was missing. Replace the missing rubber.ii. The stool in the toddler room leading to the diaper change station was broken and taped together. Replace the stool.

      • There were chemicals stored within the cupboards of the OSC bathroom.There was no lock present to restrict access by the OSC children.Install a lock on the cupboards or move the chemicals to an area where the children are unable to access.

      • 1. Cubbies were not secured to the wall. Ensure all cubbies are secured.2. A handbag with long strap was observed stored on a chair in classroom for 3 to 5 years old that is accessible to client. Remove all access to objects with long straps from children.3. Tacks and staples are not securely fastened in the walls. Remove all tacks and secure staples with tapes or substitute staples with tape, Velcro, adhesive sticky tack putty, etc.
