      • All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Clean and degrease the equipment and surfaces in detail. CLEAN THE DELI IN DETAIL AND SANITIZE THE STORE SHELVES.

      • Adequate and convenient toilet facilities shall be provided. They should be properly designed, maintained, and accessible to employees at all times. THE COOKING/DELI AREA AND THE SERVING COUNTER AT THE FRONT OF THE STORE HAVE NO EXPOSED HANDSINKS. INSTALL AN EXPOSED HANDSINK IN EACH FOOD PREPARATION AND SERVING AREA. SUPERVISOR ON SITE, ALL OPEN FOOD SERVICE CEASED BY THE OWNER AT THIS TIME.

      • All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. 15 MOUSE DROPPINGS FOUND IN THE REAR STORE ROOM, 3 RAT DROPPINGS FOUND NEXT TO THE DELI COOLER, OPEN RODENT BAIT UNDER THE SHELVES ALONG THE EAST WALL, TWO GNAWED OPEN BAGS OF CHEETO'S AND RAMEN NOODLES AND ONE BOX OF CAT FOOD AND A BAG OF DOG FOOD FOUND HALF EATEN ON THE SHELVES. CLEAN UP ALL EVIDENCE OF RODENT ACTIVITY.

    • 18. NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS
      • All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. 1/4" to 1/2" gaps found in the front and rear doors, around the pipe going out the rear screened in vent, at the bottom of the rear area door with rub marks, and the hole behind the toilet and in the register area. Remove scattered mouse droppings and open bait from the interior of the store under shelves.

      • When toilet and lavatory facilities are provided for the patrons of food establishments, such facilities shall be adequate in number, convenient, accessible, properly designed, and installed according to the municipal code. No hot water on premises for employees to was there hands, restore the hot water.

      • The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Replace all broken tiles in the store so the floors are smooth and sanitary.
    • 35. WALLS, CEILINGS, ATTACHED EQUIPMENT CONSTRUCTED PER CODE: GOOD REPAIR, SURFACES CLEAN AND DUST-LESS CLEANING METHODS
      • The walls and ceilings shall be in good repair and easily cleaned. Reseal the bathroom walls so they are smooth and sanitary from top to bottom.
    • 38. VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED
      • Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Direct the water draining from the deli cooler down a drain.
    • 41. PREMISES MAINTAINED FREE OF LITTER, UNNECESSARY ARTICLES, CLEANING EQUIPMENT PROPERLY STORED
      • All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. Remove all clutter from the store so it can be inspected.
