    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures for skin cleansing and client care in case of accidental cut were not completed. AHS templates were provided onsite to complete and return to PHI via email.
    • 20. Is reusable equipment disinfected properly?
      • 3-No HLD testing log/records were available for review.Ensure the HLD is being tested everyday prior to use for disinfection of semi=critical tools and testing records are maintained on HLD testing log sheet. Sheet provided during inspection.

      • The reprocessing sink was reportedly being used for hand washing. Operator will post a sign on the sink "No Hand washing" and remove the soap and paper towel from the sink. The sink in the washroom will be used for hand washing.

      • The floor tiles were noted broken at the entrance to the waxing room near the door and by manicure stations. Operator has plans to repair over the holidays this year.

Ensure the HLD is being tested everyday prior to use for disinfection of semi=critical tools and testing records are maintained on HLD testing log sheet. Sheet provided during inspection.

      • Cleaner/disinfectant available for the surfaces did not have DIN. Selecting a disinfectant for equipment was discussed with the help of AHS resource. Printed material provided for future reference.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The sheets and blanket on the waxing station were noted being non-sanitizable and absorbent. Reportedly not being laundered after each client. Discussed with operator and decided that the covers will not be provided to each client and will be laundered after each use.Ensure all surfaces in the facility are cleanable, impervious to moisture and in good repair.
