- A lot of products have been re-packaged into bags with no labels to indicate what it is, the source, or when it was packaged. Operator claims that some of the food products in the small freezer near the large walk ins are staff foods. There was no label or sign or any way to indicate what is for sale or what is for staff. Provide a complete list of all food suppliers and 3 months of invoices for all foods. Store all staff food items away from food for sale. It must be clearly labeled and not accessible to the public.

- Operator is re-packaging/processing bulk meat and produce on site, but is not permitted to do any food handling. Do not re-package, process, or handle food in any way. Food must be sold in the packaging that it came in.If operator wants to be able to do any additional food handling, they must notify AHS, discuss plans, submit floor plans for the area where the processing will happen, and come up with a food safety plan to be reviewed by the PHI.

- There was a van parked in the back loading dock that was a non-refrigerated van and was transporting eggs. Due to the cold outside temperatures at the time of the inspection the eggs were not in the temperature danger zone, but operator was told again that he must use refrigerated vans to maintain temperatures for eggs, meat and cut produce.

- 1. Operator is re-packaging/processing foods, which is in violation of permit conditions. Permit conditions state that operator can only sell pre-packaged foods or whole produce.2. Operators have been using un permitted vehicles to do parking lot sales of food from this store. Operator must use ONLY permitted vehicles for sales.

- Product in the refrigerators and freezers were stored haphazardly. Boxes of meat/produce were. Properly organize all refrigerated/freezer units.Provide a written sanitation plan.

- Operators have been consistently, despite being told not to several times, using un permitted vehicles to do parking lot sales of food from this store. Meat and eggs are being transported in non-refrigerated vehicles despite there being two permitted reefer vans affiliated with this location. Operator must use ONLY permitted vehicles, and must use refrigerated vans to transport perishable, high risk foods such as meat, eggs, and cut vegetables and fruit.

- There were boxes of food items, containing Lychee fruits, found during the inspection that showed mold growth. The food items were discarded during the inspection. The inspector informed the operator that a system must be in placed that monitors the condition of all food items, and that they must be discarded immediately and not be served.

The inspector informed the operator that all food items must be stored off the floor with at least six inches clearance. This would also facilitate adequate cleaning of the space underneath. Please re-arrange the walk-in unit to ensure that all items are stored off the floor.**STORE ALL ITEMS ABOVE THE FLOOR.

- Bags of raw meat were measured at 10°C in the north walk-in cooler. The operator mentioned that the bags were placed there about an hour ago. The walk-in cooler was not able to maintain adequate temperature as the surfaces of the fruits and vegetables were also measured at 10°C. Please configure the cooling unit to ensure that it is fully functional and able to maintain a temperature of 4°C at all times.**FIX NORTH WALK-IN COOLER TEMPERATURE.The south walk-in cooler showed a thick ice accumulation around the walk-in cooler condenser unit. This would not allow the proper circulation of air in the walk-in cooler. The walk-in cooler temperature was measured at 11°C. The inspector informed the operator that the walk-in cooler must be kept at or below 4°C. Please fix the walk-in cooler temperature.**FIX SOUTH WALK-IN COOLER TEMPERATURE.

The inspector informed the operator that, if the backdoor is to be kept open, it must be equipped with a tight-fitting screen door to prevent the potential entry of pests and vermin into the facility. Otherwise, the backdoor, and any other potential source of entry into the facility, must be kept closed.The front door entrance was also kept open during the inspection. The glass of the door was fixed with plywood as the glass was noted being broken. According to the operator, a recent break-in happened, and they were still in the process of getting the door fixed. Please ensure to fix the door so that it is able to be fully closed during operations.Failure to keep entry/exit points closed would allow the potential entry of pests and vermin into the facility.
