- * Visibly dirty brushes, scissors, tweezers were observed in the waxing room inside a glass cup. Corrective actions:- Clean and disinfect all reusable equipment.- Ensure used, dirty equipment is always taken to the reprocessing area to be cleaned and disinfected.- Suggest having containers with "dirty/used" and "clean" labels to prevent cross contamination between clean and dirty equimpent.

- HLD testing record was not maintained properly. The HLD sheet was not changed with new pour. Use and entering record of HLD testing was discussed onsite with owner.A new HLD sheet should be used with new pour.

- Sheets/towels blankets were observed on the waxing bed/chairs in the waxing room, reported not being laundered after each client. All the linen were removed from the waxing chairs.Ensure the surfaces of the bed/chairs are impervious to moisture, cleanable and in good repair.

- - Bubbles were observed in the high-level disinfectant PreEmpt CS20. Discarded and refilled onsite.-Manufacturer instructions for use (MIFU) of high-level disinfectant were not being followed. Tools were soaked in the high-level disinfectant for 15 min instead of 20 min required contact time. Corrected onsite after reading MIFU onsite.-The high-level disinfectant was being used beyond 14 days re-use period. Corrected onsite after reading the label of the bottle.-HLD testing logs records were not available for review.

- The glass and table surfaces of the nail stations were noted unsanitary condition. Collection of dust and stains were noted on the glass and on the surfaces of tables.Though cleaning of the tables and their glass is required.

- The reprocessing sink is located in the staff room and observed being used for washing dishes and hand washing.The hand washing sink located at facility at the very end of the front area was designated for reprocessing and clients will use handsink of the bathroom for handwashing.
