- Paper towel dispenser at handwashing sink across from the cooking line is in disrepair and not dispensing paper towels. - Please fix or replace. All handwashing sinks must be supplied with paper towels and soap in suitable dispensers.

- Inadequate pest control measures. No glue board observed 3 of 5 tin cat traps.- Please have pest control service provider to install glue boards in each trap. Recommend adding more traps as increase pest activity has been observed in the area recently.

- Several drain flies were found by the cafe area. Implement additional effective methods measures to eliminate the presence of drain flies.

- 1) Not all food handlers have proper hair control.All food handlers must have proper hair control.2) Staff with long painted nails was planning to make a milkshake without gloves. No nail brush available to wash hands with long fingernails. Staff with long fingernails must wash hands with nail brush before putting gloves on. 1 to 2) Please practice good personal hygiene.

- Quat sanitizer in the spray bottles were measured greater 400ppm. Use 200ppm quat sanitizer and verify with test strips.
