    • R4.5 - Are incidents and notifications reported and records retained as required?
      • Observation(s): The Licensee does not have a system in place to ensure that all reportable incidents are reported to Licensing. It was noted that a reportable incident that occurred in 2026 had not been reported to Licensing. If all reportable incidents are not submitted, Licensing may not be able to assess level of risk to persons in care as required which is a risk to the health, safety or dignity of persons in care. The Licensee will submit a Compliance Plan by May 13, 2026 outlining the systems and monitoring that will be put into place to ensure all reportable incidents are submitted to Licensing in compliance with the legislation on an ongoing basis. The Licensee does not have a system in place to ensure that records of reportable incidents are retained. It was noted that copies of some reportable incidents could not be located. If records of reportable incidents are not kept the Licensee and/or Licensing may not be able to follow-up as required which is a risk to the health, safety or dignity of persons in care. The Licensee will submit a Compliance Plan by May 13, 2026 outlining the systems and monitoring that will be put into place to ensure that copies of all reportable incidents are retained in compliance with the legislation on an ongoing basis.
      • R4.5G - Immediately notify licensing in the form and manner required, if a person in care is involved in a reportable incident; 77( 2 )(c)
      • R4.5M - Retain a record of reportable incidents involving persons in care; 88(c)
    • R1.1 - Does the Licensee operate in accordance with their license and inform Licensing of any significant changes?
      • Observation(s): The Licensee does not have a system in place to ensure that a change in manager is reported to Licensing immediately. There was a change in manger in 2025 that was not reported to Licensing and was identified by Licensing while following up on a reportable incident. If a change in manager is not reported to Licensing there may be a delay in Licensing being able to make contact during an emergent situation which increases the risk to the health, safety or dignity of persons in care. The Licensee will submit a Compliance Plan by May 13, 2026 outlining the systems and monitoring that will be put into place to ensure that changes in manager are reported in compliance with the legislation on an ongoing basis.
      • R1.1E - Notify licensing if the manager resigns or expects to be absent for at least 30 consecutive days; 8( 3 )(a)
    • R2.1 - Are written policies and procedures in place to guide employees in all matters relating to the care and supervision of persons in care?
      • Observation(s): The Licensee reported that they are currently reviewing their Code Red procedures but there is no system in place to ensure that fire drills are conducted regularly. There is a risk to the safety of persons in care when each employee is not trained in the implementation of the emergency plan. The Licensee will submit a Compliance Plan by May 13, 2026 outlining the systems and monitoring that will be put into place to ensure that all employees are trained in the implementation of the emergency plan in compliance with the legislation on an ongoing basis.
      • R2.1F - Emergency plans must set out procedures to prepare for, mitigate, respond to and recover from any emergency including evacuation procedures; 51( 1 )(a)
    • R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
      • Observation(s): The Licensee does not have a system in place to ensure that persons in care’s care plans are current. It was noted that a care plan had an outdated directive and that there were differing versions of the care plan in use. If care plan(s) are not current and consistent, care may be provided which does not meet the needs of the person(s) in care therefore, it is a risk to their health, safety or dignity. The Licensee will submit a Compliance Plan by May 13, 2026 outlining the systems and monitoring that will be put into place to ensure that care plans for all persons in care are current and consistent in compliance with the legislation on an ongoing basis.
      • R10.3K - Review and, if necessary, modify each care plan if there is a substantial change in the circumstances of the person in care, or at least once a year, to ensure it continues to meet the needs, preferences, and is compatible with the abilities of the person in care; 81( 4 )(b)(i)(ii)
