- 301 - Critical Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
- Observation: Observed build up of organic debris and grime within the internal compartments of the ice machine equipment. Organic debris, grime and unknown residues can directly contaminate ice products resulting in potential foodborne illness.
- Corrective Action: Operator to remove all organic debris, grime and residues from the equipment, thoroughly clean and then sanitize the internal compartments of the ice machine equipment. Operator is prohibited from utilizing the equipment until granted verbal permission by the environmental health officer.
- 302 - Critical Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
- Observation: Inadequate sanitizing rinse solution. No sanitizer detected. (Public Health significance) - Automatic glass dish washers must provide a minimum concentration of 12.5 parts per million (ppm) as required to effectively sanitize glassware.
- Corrective Action: Immediately initiate hand sanitizing providing a 200 ppm solution of QUAT and soak for 2 minutes for all glassware. Have the glass dishwasher repaired by a qualified technician and monitor and log sanitizer concentration regularly so you know when the unit needs adjustment or repair.

- 209 - Food not protected from contamination [s. 12(a)]
- Observation: Food items (including beverages) in the kitchen area as well as the walk-in cooler were stored on the ground (Public Health significance) - Storage areas should have adequate shelving such that all materials, including food and food items, are maintained at a minimum of 15 cm (6 in.) off the floor on racks, shelves or pallets. Contamination of food from contact with objects, people, pests or chemicals can cause serious illness.
- Corrective Action: Ensure all food items are placed on racks and shelves which are at least 15 cm (6 in.) off the ground to prevent contamination of food as well as to allow for cleaning.
- 305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
- Observation: Back door has a gap that can allow mice and pests to enter. Mice can fit through gaps as small as a pencil tip. Public Health Significance - Mice feces can carry disease and contaminate food that can lead to foodborne illness.
- Corrective Action: The operator must ensure back door is properly sealed to prevent entry of pests.

- 308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
- Observation: Glass washer sanitizer (iodine) concentration was not detectable. Proper iodine concentration should be 12.5-25ppm during the sanitize cycle.
- Corrective Action: Discontinue use of this machine until repairs are complete and sanitizer is dispensed at the appropriate concentrations. Use single use cups or use the other dish machine for glassware in the interim. Sanitizer concentrations must be monitored (eg. with test strips) at least daily.
- 308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
