- A gap was observed at the back door. Please ensure that the back door is sealed/weather stripped to prevent potential pest entry. Also ensure to have to have a pest control checklist on-site and facility should be monitored every month. - IN PROCESS1) The Dry Storage area as well as the Back Room containing the Freezers was lined with Carpet at the time of Inspection.2) CORRECTED: Some of the Tiles in the dishwashing area were damaged/worn out at the time of Inspection.1 - Ensure that the Carpet is removed and replaced with a flooring that is smooth, non-absorbent, and easy to clean in order to facilitate proper cleaning and sanitizing of said flooring. The Dry Storage area may remain carpeted if all Food in the Dry Storage area is moved off of the Storage Shelves, and into an area that has non-absorbent, easy to clean Flooring.

- Observed fake nails on the food handler's hand. Please ensure to wear gloves while handling food, Although, it is a best practice to forgo the nails. At the end of the inspection PHI ensured food handler wears gloves that are in good condition.
