    • R10.4 - Are restraint and fall prevention plans appropriate?
      • Observation(s): When reviewing a care plan for two hour restraint monitoring, observed several instances of monitoring documentation missing. Lack of restraint monitoring has potential to physically and emotionally harm persons in care. Submit a corrective action plan by May 1, 2026, indicating how the licensee will ensure a system is in place for restraint monitoring throughout the use of the restraint and assessed after use of restraint.
      • R10.4C - Monitor the safety and physical and emotional dignity of the person in care throughout the use of the restraint and assessed after the use of the restraint, 73( 1 )(c)
    • R6.3 - Does the facility demonstrate appropriate outbreak prevention and control measures?
      • Observation(s): Nail clippers observed loose in spa room drawer. When nail clippers are not kept separately for each person in care (PIC), there is risk of communicable disease between PIC. Submit a corrective action plan by May 1, 2026, indicating how the licensee will ensure a system is in place to have all nail clippers and other personal use items stored in a manner to maintain health and hygiene for all PIC.

    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): Observed access door to servery unlocked in one dining room area. In another dining room area, observed staff leave door open and severy assesible to PIC. When PIC have access to the servery, they may be exposed to stoves, sharps, and other potentially hazardous items which could lead to injury or illness. Submit a corrective action plan by May 1, 2026, indicating how the licensee will ensure a system is in place to have hazardous materials inaccessible to persons in care.
      • R7.1AK- Provide appropriately furnished and equipped areas for secure, safe and adequate storage of cleaning agents, chemical products and other hazardous materials; 35( 1 )(c)

    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): The system to ensure that all medications in the community care facility are safely and securely stored is ineffective. A prescription treatment cream was observed in an unlocked cupboard in one person in care's bathroom.

    • R8.1 - Is there an ongoing planned program of physical, social and recreational activities?
      • Observation(s): A review of care plans, recreation participation statistics and an interview with staff indicates that recreation programming available to PICs may not meet their needs. (for example, there is not formalized 1:1 recreation programming.)
      • R8.1A - Provide a program of activities, without charge, that is suitable to the needs of persons in care (Does not apply to Hospice); 55( 1 )(a)(i)

    • R3.1 - Do the manager and employees meet the employment requirements and are they assessed regularly for performance?
      • Observation(s): Licensing reviewed a sample of employee records. Employees hired since the change of licensee obtained RCMP vulnerable sector criminal record checks prior to employment. Please ensure all employees obtain clear criminal records checks under the Criminal Record Review Act.
      • R3.1B - Ensure criminal record checks are obtained for all employed persons; 37( 1 )(a)
