Templates for the above procedures will be emailed to the operator.ACTION REQUIRED:1) Establish a written procedure for skin cleansing prior to waxing and eyelash extension.2) Establish a written procedure for client care in the event of accidental skin cut. Also purchase a first aid kit if you have not already done so.

      • 1. Observed high level disinfectant test strips had expired. Please obtain new preempt CS20 test strips.2. High level disinfectant testing log sheet was not observed. A copy of the log sheet was given to the operator during inspection. Maintain a testing log-sheet for high-level disinfectant used at the facility. Poured high-level disinfectant must be tested each day used and discarded after 14 days (according to the manufacturer's instructions) or when the testing fails.

      • Used wax dipping stick was observed on top of the wax bowl. The stick was discarded during inspection.Please ensure single deep method is always implemented and used stick disposed immediately after use.
