    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • (1) CDI-A personal services notification had not been submitted. The operator submitted a PSN during the inspection.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • (1) CDI: There were no written procedures available. The operator completed (i) The Written Procedure for Cleansing Skin during the inspection. See copy attached to notes.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • (1) There was no washing step being done for the eyelash tweezers. The operator was wiping the tweezers with CaviWipes then immersing them in PREempt CS20. * The clean-up area set-up was discussed. Ensure it contains (i) an area for washing tools with hot soapy water. Recommend using a brush to scrub tools clean. (ii) Rinsing step with clean running water. (iii) Air dry tools on towel or rack. (iv) Immerse tools in disinfectant solution for the required contact time. 20 minutes for PREempt CS20. (v) Rinse tools and dry before storing.
    • 20. Is reusable equipment disinfected properly?
      • (1) The operator was wiping the tweezers with CaviWipes then immersing them in PREempt CS20. (2) CDI: There were no High Level Disinfectant Logs being kept. The operator started the High Level Disinfectant log form during the inspection but did not have test strips to complete the test.(3) There were no AHP7 test strips available for testing the high level disinfectant (PREempt CS20). The operator ordered test strips from Amazon during the inspection. They are due to arrive on Saturday, April 13.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • (1) There was no reprocessing sink available for the semi-critical instruments (eyelash tweezers). Tools were being processed inside the service room. There is no sink available in this area.
