      • Facility was found to be offering services (neuromodulator injection, low energy-emitting equipment) onsite via operators renting service rooms. There is no record of notification to AHS regarding these additional personal services.Ensure that notification is given of any new personal services offered onsite.

      • One of the stored clean tweezers in the eye lash services room appeared to have a build-up of residue or debris.Tweezer was removed from storage during inspection to be reprocessed.

      • Test strips to verify the strength of CS20 high-level disinfectant being re-used was not observed onsite at the time of inspection. The testing of CS20 was not being recorded on log sheets - only the replacement of high-level disinfectant was indicated. Provide test strips for high-level disinfectant in use to verify strength during re-use period.Record the results from high-level disinfectant testing for any day in which tools are being disinfected, as specified by the disinfectant manufacturer's instructions.

      • Handwashing was observed to be conducted in a sink actively being used for equipment reprocessing.Handwashing supplies were observed in proximity of sinks in service rooms identified as being used for tool reprocessing.Do not use reprocessing sinks for handwashing and ensure that handwashing sinks are not also being used to wash tools. Remove handwashing supplies from sinks intended for dedicated reprocessing, and remove reprocessing supplies from sinks intended for handwashing only. Alternatively, provide a dual purpose sink procedure that describes the decontamination process of the sink in between handwashing and reprocessing uses.
