    • R2.1 - Are written policies and procedures in place to guide employees in all matters relating to the care and supervision of persons in care?
      • Observation(s): The system to ensure that all policy and procedures are reviewed yearly is ineffective. A review of the Wandering residents’ policy was last revised in May 2022. All policy and procedures should be current as these are guidelines for all staff to implement. Submit by October 10, 2025, the plan that will be implemented to ensure ongoing compliance with Section 85(1)(b) of the Residential Care Regulation.
      • R2.1Q - Review and, if necessary, revise policies and procedures at least once a year; 85( 1 )(b)
    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): The system to ensure that all rooms and common areas are maintained in a safe and clean condition is ineffective. it was observed that an outdoor shed containing gardening tools was unlocked, posing a safety risk. It was also observed that sections of the pathway in the garden were uneven posing a tripping and falls risk. Not maintaining a safe environment poses a potential safety risk to persons in care. Submit by October 10, 2025, the plan that will be implemented to ensure ongoing compliance with Section 22(1)(c) of the Residential Care Regulation.

    • R6.3 - Does the facility demonstrate appropriate outbreak prevention and control measures?
      • Observation(s): The system to ensure fridge temperatures are recorded is ineffective as evident by missed days of recorded temperatures for both the kitchen fridge and bedroom fridges. This was also a contravention on the previous inspection.
      • R6.3B - Ensure that food is safely prepared, stored, served and handled; 63 ( 1 )

      • Observation(s): Fridges located in person in cares bedrooms did not have a system in place to monitor the temperature when in use.
      • R6.3B - Ensure that food is safely prepared, stored, served and handled; 63 ( 1 )
    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): Hot water tap in the dining room which is accessible to persons in care exceeds 49 degrees Celsius. It was observed that sections of the pathway in the garden were uneven posing a tripping and falls risk.
      • R7.1C - Ensure water accessible to a person in care does not exceed 49 degrees Celsius; 17

      • Observation(s): The menu did not include two nutritious snacks with at least 2 food groups described in the Canada's Food Guide.
      • R5.1D - Provide for each day, at least 2 nutritious snacks with at least 2 food groups described in Canada's Food Guide; 62( 2 )(b)

    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): The system for ensuring compliance with storage of hazardous materials is ineffective. A container of cavi-wipes was found in one of the storage areas and the storage area is accessible to persons in care. The container of cavi-wipes was removed from the storage area by the manager and secured. This item was corrected during the inspection.
      • R7.1AK- Provide appropriately furnished and equipped areas for secure, safe and adequate storage of cleaning agents, chemical products and other hazardous materials; 35( 1 )(c)

    • 22- Is personal privacy respected and records and personal information kept confidential?
      • Observation(s): As previously noted in the inspection report: A Medication Administration Record binder was found in the hallway on top of the medication cart and the information on the Medication Administration Record accessible to passers by. The medication cart and the Medication Administration Record was removed from the hallway and locked in the medication room during the inspection. The facility has two med carts and two binders containing Medication Administration Records. The second med cart with the Medication Administration Record was stored in the locked medication room. This infraction was corrected during the inspection.
      • Respect personal privacy and keep records and personal information confidential.
    • 4.1 Are person in care records current, complete and kept confidential?
      • Observation(s): There are no written consents on file to call a medical or nurse practitioner or ambulance in the case of an accident or illness. A Medication Administration Record binder was found in the hallway on top of the medication cart and the information on the Medication Administration Record accessible to passers by. The medication cart and the Medication Administration Record was removed from the hallway and locked in the medication room during the inspection. The facility has two med carts and two binders containing Medication Administration Records. The second med cart with the Medication Administration Record was stored in the locked medication room. This infraction was corrected during the inspection.

      • Keep the records and personal information of persons in care confidential to the greatest extent possible while maintaining the health, safety and dignity of persons in care; 93

    • 2.1 Are written policies and procedures in place to guide employees in all matters relating to the care and supervision of persons in care?
      • Observation(s): Reportable incident policy did not contain "aggression between persons in care" and still had the two page incident report form included in the policy manual.
      • Review and, if necessary, revise policies and procedures at least once a year; 85(1)(b)
    • 7.1 Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): There were no fire inspection or sprinkler inspection reports on record since 2013. The facility has their own emergency equipment checking schedule in place but the fire alarm and sprinkler system has not been professionally tested since 2013. During the inspection the facility had arranged appointments for fire and sprinkler testing to be completed by the end of the month. Thermometers were noted in the medication fridge but temperatures were noted to be taken and logged only once a week. There were several types of medications being stored with various temperature storage ranges. The most temperature sensitive medication was on the insulin being stored in the med fridge. Safe storage and temperature monitoring was discussed with the manager. The facility had been informed by a person within the Health Authority that a special thermometer is to be installed in the medication fridge and temperature monitoring to increase to twice a day. The facility is in the process of completing the Health Authorities request.
      • Inspect and maintain on a regular basis all rooms and common areas, emergency exits, equipment, and monitoring and signalling devices; 22 (3)
