- - Creative Minds Room: Children jackets and backpacks were stored on hooks without proper separation, and the personal items were overlapping/touching. **Please install dividers, place personalized items in labeled bins with lids, utilize tote bags, separate the hooks 6 inches apart or utilize other means to keep the children's personal belongings from touching to prevent germ /lice/ bed bug transmission etc. ** Dec 12 2025 discussion: Director explained that large tote bags will be used and have already been purchased. Children's personal belongings will be put in the tote bags and hung up. This would be satisfactory as this prevents children's personal belongings from touching** -

Creative Minds Room: The stand up lamp had it's long electrical wires dangling , which may become a choking hazard.** Please secure the dangling electrical wires to eliminate the choking hazard. **This was corrected at the time of the inspection. 3. Mighty Champs Room: There was a lightweight canvas that was falling off from the wall, that was being held by 2 thumb tacks. ** Please do not use thumb tacks- secure the canvas to the wall through more secured means. ** The canvas was put on the floor and thumb tacks removed - the violation was corrected at the time of the inspection. -

- - Mighty Champs Room: There was a large storage bin labeled 'clean toy bin' that was stored directly underneath the paper towel dispenser on the counter. The location of the bin obstructed the ability to get the paper towels. ** Please relocate the storage bin and keep the handwashing station/ paper towel dispenser clear from any obstructions to encourage proper handwashing. ** The operator moved the bin and this was corrected at the time of the inspection** -

Mighty Champs Room: The antiseptic wipes in the first aid kit was expired in Jan of 2025. ** Please replace the antiseptic wipes with new wipes that are not expired. ** This was corrected at the time of the inspection. 2. There were no CPR barriers found in the first aid kits. ** Please ensure the first aid kit is equipped with a rescuscitation barrier device with a one-way valve. -

Mighty Champs: The diapering mat was torn on the surface, revealing the absorbent soft cushioning material underneath. **Please replace the diapering mat with a new one that is in good condition and easy to clean. ** This was corrected during the inspection **2. Mighty Champs: There was a toilet seat that had dried/ accumulated urine/feces staining observed on the surfaces throughout that was stored beside the toilet. Operator mentioned that this toilet seat was for a child that no longer attends the site. ** Parent provided toilet seats should be labeled with the child's name , stored appropriately, and cleaned/disinfected regularly or sent home for cleaning. ** Operator disposed of the toilet seat - violation was corrected **-

- Paper towel in the Mighty Champ, Curious Minds, and Creative Explorer classrooms was not located inside a suitable dispenser. This was corrected during the inspection.REQUIREMENT: Paper towel must be kept inside a suitable dispenser to ensure the bulk supply is maintained in a sanitary condition.

- 1) The light bulb switch in the Curious Minds classroom had caked on debris; this was corrected during the inspection.REQUIREMENT: Ensure light bulb switches and other high-tough surfaces are routinely and thoroughly cleaned then sanitized.2) In the Mighty Champs room, the diaper changing mat was highly damaged with tears and rips all over the mat. This mat was replaced with the mat from the Little Beams room since that room was not in use and the mat was in good repair.REQUIREMENT: Ensure that all diaper changing mats are in good repair and easy to clean.

- Several mats, including the diamond mat in the Little Beams classroom, and the striped mat in the Mighty Champs classroom, had higher amounts of debris which indicated infrequent or not thorough cleaning. The operators indicated that mats are supposed to be vacuumed on a daily basis from a 3rd party company.REQUIREMENT: Ensure that all surfaces in the daycare are routinely and thoroughly cleaned.

- In the Mighty Champs classroom, bins labelled 'Mouthed Toys' (i.e. toys that need cleaning) also had a 'Clean Toys' label, making it confusing whether toys were clean or contaminated.This was corrected by removing the 'Clean Toys' label.REQUIREMENT: Ensure all items which require a label for clarity purposes do not also have a contradictory label.

- Sanitizer concentration in several rooms was measured to be between 10-50 ppm.REQUIREMENT: Maintain chlorine bleach sanitizer at a concentration of 100 ppm.

- Handwashing sinks located in the 4+ year old room had a temperature which was measured to be between 61-62C at the maximum.REQUIREMENT: Maintain the maximum temperature at handwashing sinks no more than 54C and recommend no more than 49C.

- Fogger filled with bleach solution was measured to be well above 1000+ ppm. Operators were using this tool for toy disinfection. There are several concerns to this method:1) Aerosolizing any disinfectant can irritate the skin, eyes, or airways and can cause other health issues for people who breathe it in.2) 1000+ ppm bleach cannot be reliably controlled in it's application using a fogger compared to a spray bottle or sink submersion for toy disinfection.3) 1000+ppm bleach would not be appropriate for use in the kitchen and the food-contact surfaces it could encounter.The operators discarded the bleach solution and stated they will not use this method for the future.
