- 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
- 1)There is no paper towel dispenser or paper towel available for back hand washing sink.2)What was originally used as a handwashing sink in the back area is now used as a preparation sink. Paper towel and soap were removed. The designated handwashing sink is now the third compartment of the dishwashing sink, however the short faucet for the sink does not allow for proper handwashing. 3)Paper towel in dispenser at the front end hand wash sink was not dispensing properly. Paper towel was ripping in half when coming out of dispenser and could not be used to dry hands. Action taken /required1)Proper paper towel dispenser with paper towel must be accessible in the back food preparation area.2)Faucet must be replaced to allow the 3rd compartment of the 3 compartment sink to be used as the back hand washing sink.

- Reinspection - February 13, 2024Pest reports have shown reduced number of cockroaches during previous week's treatments. However, during inspection several adult live cockroaches and numerous other dead were found in the areas beside the small beverage fridge and the extension cords against the wall directly adjacent to the door between the front and back of the facility. Dead cockroaches were found on the back of the test strip holder above the 3 comp sink, on the back of an old wall poster, inside the extension cords and modem. February 6, 2024There was a significant cockroach infestation in the food establishment with live and dead cockroaches of various life cycles on pest control glue boards The largest concentration of activity was found under the 3-compartment sink and under the front water well.

- While pest control records were available with the mall's operation manager, Ryan, past/current records were not available onsite in the facility. Owner and staff were not provided, in any manner, details of bi-monthly inspection/treatment from pest control company.
- 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
- Numerous electrical cords, computer items (modem) and other non food related items are tangled and grouped together adjacent to the door between front and back of restaurant. Cords and equipment cannot be properly cleaned and are creating a harborage for cockroaches. Action Required/TakenCords and equipment must be thoroughly examined and cleaned to ensure there are no further evidence of either dead or live cockroaches. They then must be organized and stored in a manner that does not create a harborage for pests and in a way they can be cleaned.

- Food handler involved in open high risk food preparation has long finger nails which are painted with dark polish. Action Taken /RequiredFood handler immediately put on gloves. Long finger nails coated in dark nail polish cannot be properly cleaned by hand washing and can hide dirt/debris build-up.
