    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Client records were kept, however sterility assurance documentation for critical equipment used during microneedling services were not collected and retained.Corrective Actions:1. Collect the sterility assurance documentation for all critical equipment used (microneedle cartridges) with each client's records.2. Once microneedling services resume, please email PHI a copy of the sterility documentation for your cartridges and the corresponding client record. PHI strongly recommends stapling or attaching the sterility assurance documentation directly to the applicable client records.

      • Client records were kept, however sterility assurance documentation for critical equipment used during microneedling services were not collected and retained.Corrective Actions:- Collect the sterility assurance documentation for all critical equipment used (microneedle cartridges) with each client's records.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Impellers inside the pedicure foot basins were not disassembled to be cleaned and disinfected between each client.Once they were opened by the PHI, there was buildup of debris and hair within the impeller.Corrective Action:Ensure the pedicure basin impellers are disassembled, cleaned and disinfected with a low or intermediate level disinfectant after each client service.
    • 20. Is reusable equipment disinfected properly?
      • Pedicure and manicure equipment were not disinfected. Instead, they were cleaned and placed in a UV cabinet, which is not an acceptable method of disinfection.All disinfectants in the reprocessing area were expired, namely:- Preempt CS20 (expired November 9, 2024)- Preempt RTU (expired 04/2025 and 09/2022)Test strips required to test the efficacy of the Preempt CS20 were not available. Log sheets containing test results were last used August 2024.Halo head spa unit was designed to recirculate used water from the sink, through the halo ring and back onto the client. The piping and halo ring cannot be adequately cleaned and disinfected. Corrective Actions:- Obtain new disinfectants that are not expired (Preempt CS20, Preempt RTU)- Obtain test strips (AHP 7)- Follow the manufacturer's instructions for use for all disinfectants, including reuse periods, contact time, and testing.- Test the CS20 disinfectant each day it is used, and document the results in a logsheet.- Between each client service, run the unit with undiluted Preempt RTU disinfectant for 3 minutes, followed by a rinse with clean water to ensure the removal of all disinfectant from the pipes.
    • 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
      • *There were no manufacturer's instructions available for a new piece of equipment; staff member on-site indicated that it was a "carbon laser facial." Please ensure instructions are available on-site.This piece of equipment cannot be used until manufacturer's instructions are provided to Environmental Public Health.
