    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • There are no written procedures for an accidental cut or puncture or decontamination of a hand sink used for equipment reprocessing at this facility. PHI provided written procedure templates for both processes to be filled out by the operator with the inspection report.
    • 20. Is reusable equipment disinfected properly?
      • There are no test strips or testing records of a high-level disinfectant. Ensure that test strips are obtained for the chosen disinfectant and that a record log is kept for testing and verification of the concentration of the high-level disinfectant.
    • 20. Is reusable equipment disinfected properly?
      • Semi-critical equipment (including, but not limited to cuticle pushers) are not being disinfected with an appropriate high level disinfectant. During the inspection it was identified that the operator was using 99% isopropyl alcohol on semi-critical equipment. TEMPORARY SOLUTION:Switch to single use disposable cuticle pushers until an appropriate high-level disinfectant is acquired. LONG TERM SOLUTION:Obtain an appropriate high-level disinfectant that contains a drug identification number (DIN) and that it is used based on the manufacturer's instructions.
