- 1. The children’s table in Room 4 was damaged, with raw wood exposed, making it difficult to clean and sanitize effectively.Refinish or repair the table so all surfaces are smooth, sealed, and washable. 2. The mirror in Room 4 was damaged and torn. The operator has a replacement mirror available in the office, but it has not yet been installed.

- 06. Are proper protocols being followed in regards to client care?
- 1. While observing diapering procedures in Room 1, the staff member did not change their gloves after removing the soiled diaper. The same pair of gloves was used to put on a clean diaper and dress the child. Education was provided during the inspection. Staff were advised that new gloves should be put on after the soiled diaper is removed and before handling clean items or dressing the child. Proper glove use helps prevent the spread of contamination and maintains appropriate hygiene standards.2. The bleach solution at the diapering station in Room 3 measured 10 ppm when tested. A new solution was prepared during the inspection.

- 1. One of the bathroom handwashing sinks in Room 6 did not have soap available at the time of inspection. A bottle of soap was placed back at the sink during the inspection.

- 1. Several quat sanitizer bottles tested in Rooms 3, 4, and 6 measured below 200 ppm. The bottles were refilled and retested during the inspection, and all measured 200 ppm after refilling. The sanitizer concentration in the bottles may have decreased over time, resulting in inadequate strength at the time of testing.
