      • One of the hot holding units used for rice noodles was not properly functioning and holding temperature above 60C. Informed operator to repair or replace unit or ensure rice noodles are kept refrigerated.

      • Thermometers were missing for the two sandwich prep cooling units. The digital display on the cooling units and monitored temperature had an error code. Please ensure thermometers are available inside these units or digital display is properly working.

      • The low temperature chlorine chemical dishwasher was not properly functioning with the sanitizer line disconnected and unable to dispense the solution throughout the machine. Operator attempted to reconnect the line but was unsuccessful. Please ensure corrective actions are taken and the chemical dishwasher is properly functioning and sanitizer is properly dispensing.Informed operator to temporarily use the dishwasher to wash and rinse and the two compartment sink to sanitize utensils and equipment until the dishwasher is repaired.

      • No chlorine test strips were available onsite. Please ensure valid chlorine test strips are ordered and available onsite.

      • No soap or paper towels were initially available for the front counter handwashing sink. Informed operator who provided soap and paper towels. The rear handwashing sink in the kitchen was not stocked with paper towels. Informed operator who provided paper towels to the station.Please ensure handwashing stations are fully stocked at all times.

      • The handwashing station at the rear of the kitchen was slow to drain. Please ensure corrective actions are taken and water in the handwashing basin is properly draining.

      • No recent self monitoring pest control records were completed. Please ensure corrective actions are taken and pest control records completed and up to date.

      • The posted food handling permit was expired. Please ensure a new valid food handling permit is posted onsite.

      • The exhaust hood service was overdue. Please ensure the exhaust hood is cleaned and serviced and routine cleaning is conducted as per the expected frequency.

      • Walk in cooler had a temperature of 12C. There was no meats stored but cooked rice was present. Until the cooler can be maintained below 4C ensure no meats or rice are stored there. Only lower risk products like vegetables, drinks and sauces.

      • Used cleaning cloths were left on the counter/food contact surfaces when not in use. CDI: submerged in sanitizer solution at the time of inspection.       • 1. Several foods were thawing at room temperature. Corrected without instruction. Moved to walk-in freezer & prep table coolers.2. Raw eggs were stored in the malfunctioning walk-in cooler. ACTION REQUIRED: do not store eggs or other high-risk foods in the walk-in cooler until repaired.3. Unable to locate the probe thermometer. ACTION REQUIRED: obtain a probe thermometer.

      • 2025-05-12Mouse droppings & nesting materials (insulation) observed around the hot water tank. Facility is self-monitoring/treating the mouse infestation. Several traps set throughout the facility. No written records of pest control activities available. __________________2024-06-24Mouse droppings had been cleaned up. Facility is still self-monitoring for pest activity. Discussed requirement for maintaining pest control records. The snap trap by the exterior back door was broken - recommended installing a more durable style trap to meet the demands of the facility. Several (~5) dead mice in the trap behind the walk-in cooler. Action required: contact landlord & neighboring business. Implement a coordinated approach to control pest activity within the building complex. _____________________2024-06-03There were mouse droppings present in multiple areas of the facility; Infront of exterior door, food storage area, and under the shelves at entrance door to kitchen area. Do not store any high-risk foods (meat, eggs, open sauces, etc.) in the cooler until repaired.

Scoops with handles are available for dispensing food and are stored outside the containers. Remove all personal use items from food area and store them in a separate are intended for personal use.

      • During the start of the inspection used cleaning clothes were observed on the counter.Operator removed them from the counter and placed them in the sanitizing pail, before being instructed by PHI.

      • -The small prep cooler across from the deep fryer was measured at 9 degrees C.-The stainless steel cooler by the front counter was measured at 8 degrees C.Coolers must be able to maintain food at a temperature of 4 degrees C or less to prevent rapid germ growth. You must adjust or repair these coolers. -Thermometers for coolers and freezers were not available. These must be obtained and used regularly to monitor and ensure correct temperatures are maintained.

      • -Traps for monitoring pests such as cockroaches were not available. Sticky traps/glue traps are recommended. Obtain these and place in various locations such as under the dishwashing sinks in corners, under shelves, under equipment and next to exit doors.
