      • Push pins were observed in several classrooms, including the sensory room.-Remove all push pins. Use appropriate alternative materials that do not pose potential hazards.

      • Electrical outlets without cover were observed in the movement room.-Please cover with suitable materials to prevent children access to hazards.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. Torn diapering mat was observed in the facility's washroom.-Please replace with new mat with smooth, non-absorbent and easy to clean surface.2. The area surrounding the changing mat was dirty and extraneous materials including sensor bottles were observed.-Please remove all materials on the diapering station, and clean and sanitize the surrounding area to maintain sanitary condition.

      • First aid kits missing emergency blankets.-Please provide the identified item in the first aid kits.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Facility using kitchen dishwasher to clean toys. The rising temperature of the dishwasher was found to 64'C.Please repair and ensure dishwasher rinsing temperature is maintained at or above 71'C for proper cleaning and sanitizing of toys and other equipment.*In the meanwhile, please sanitized the toys in 100 ppm of bleach solution and air dry after they have washed in the dishwasher.

      • Hot water temperature was measured at 28 °C in the washroom.-Please increase and maintain the hot water temperature between 38 °C and 43 °C to support proper handwashing practices.
