- Electrical outlets without cover were observed in the movement room.-Please cover with suitable materials to prevent children access to hazards.

- 1. Torn diapering mat was observed in the facility's washroom.-Please replace with new mat with smooth, non-absorbent and easy to clean surface.2. The area surrounding the changing mat was dirty and extraneous materials including sensor bottles were observed.-Please remove all materials on the diapering station, and clean and sanitize the surrounding area to maintain sanitary condition.

The rising temperature of the dishwasher was found to 64'C.Please repair and ensure dishwasher rinsing temperature is maintained at or above 71'C for proper cleaning and sanitizing of toys and other equipment.*In the meanwhile, please sanitized the toys in 100 ppm of bleach solution and air dry after they have washed in the dishwasher.
