      • An ultrasonic cleaner was not available at the time of inspection. Operator states ultrasonic cleaner broke prior to inspection.- Ensure critical equipment is cleaned with an ultrasonic cleaner before sterilizing in an approved autoclave
    • 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
      • 1. An unapproved dry-heat sterilizer is being used to process critical equipment and jewelry. Acceptable sterilizer units must use steam to sterilize equipment.- All sterilization activities must stop as unit is not approved and cannot be used to sterilize.- All tools and jewelry processed through the dry-heat unit must not be used and must be re-sterilized in an approved unit before use.2. Physical parameters of time and temperature are not being documented - The time and temperature of an autoclave must be documented to verify sterilization parameters were reached for each load3. Chemical indicators are unverifiable on packages as unit is not using steam to sterilize- A steam autoclave must be used to sterilize. If chemical indicators are unchanged, sterilization cannot be verified.4. Chemical integrator strips are not being used for every load and the ones used do not verify sterilization as unit uses dry heat and does not - Ensure chemical integrator strips
