      • Manufacturers instructions are not being followed for disinfection. Contaminated equipment is being stored with clean equipment or single use ready to use equipment.Staff were educated on Manufacturer's instructions for use, and separation of contamination equipment.

      • Used single use items were observed in various areas of the facility. Single use equipment is to be used once and then discarded.Used single use equipment was discarded.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Contaminated equipment was being stored with clean equipment, and single use ready to use equipment. Equipment is to be kept separated. Once equipment is used it is to be stored separately in a place that cannot contaminate clean equipment and surfaces.Clean equipment is to be protected from contamination until used on clients.

      • Water was being added to high-level disinfectant. High-level disinfectant was being used for 14 days without being tested daily. Test strips and testing logs were not available. Staff were not drying items prior to placing into high level disinfectant. Manufacturer's instructs were not being followed.
