- A couple of bags, one containing chips and another, bread, were left open. This was corrected by the GM. Staff should ensure food is protected and properly stored before leaving for the day.

- At the time of inspection, it was observed that the temperature logs were being completed ahead of time. The inspection was done at 10am however the temperatures were logged for 3pm and 10pm. This is unacceptable.

- The kitchen and staff bathroom is dirty. Thorough cleaning must be done daily. There needs to be a cleaning schedule present for the bathroom and kitchen area.Walls are chipped and in disrepair which makes it harder to clean.

- 11. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Critical)?
- The iodine bottle in the glass washer in the bar is empty. Repeated Violation. Operator put Chlorine sanitizer in this empty bottle, and it was measured at 25 ppm to 50 ppm. Ensure iodine sanitizer is at 12.5 PPM ~ 25 PPM, or Chlorine sanitizer at least 50 PPM. Have a maintenance service person service this unit and contact PHI for follow-up.

- The wall across from the dishwasher and the hallway going to the dry storage area is chipped, this can no longer be cleaned. Repair and repaint the walls. The seal around the walk in freezer had duct tape on it to keep the seal in place. Duct tape is not acceptable for repair as it cannot be cleaned. Repair the seal.
