      • An uncovered electrical outlet was observed in one of the infant/toddler rooms.-Please cover all electrical outlets to prevent the risk of electrical hazards.
    • 06. Are proper protocols being followed in regards to client care?
      • Dirty diaper mats were observed in the infant washrooms.-Please clean and disinfect diaper mats after each use. Conduct regular visual checks or inspections to ensure that diapering surfaces are properly cleaned and disinfected after each diapering procedure, to prevent the spread of contamination or infection.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Dirty toys were observed in the older toddler classroom.-Please clean and sanitizer these toys as required to maintain sanitary conditions.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Children's blankets were observed touching each other in one of the infant classrooms.-Please ensure children personal items including blankets/beddings are stored without touching each other.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Bleach sanitizer bottles were not labeled in some classrooms. The concentration of the bleach solution in most classrooms ranged between 500–1000 ppm.-Please ensure all sanitizer bottles are clearly labeled with the chemical name and concentration.-Dilute and maintain bleach sanitizer at 100 ppm, and use appropriate test strips to verify the concentration.

      • Long tangling wires were observed in some facility's classrooms.-Please tag down or keep long wires out of children's reach to prevent the risk of entanglement.
    • 08. Is this facility properly constructed (Non-Critical)?
      • Some facility's classroom floors are covered entirely with rugs which does not meet the regulatory requirements.-Please provide a segment in the indicated classrooms with smooth, impervious to moisture and easy-to-clean floor surface as children's eating/food service area.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • 1. Conspicuous reinforced steel columns observed in some classrooms.-Please provide protective barrier (made of suitable material) around the indicated steel column to prevent or reduce the risk of injury.2. Some glass doors have pinch point in the joint.-Please install door pinch guards on the indicated door joints.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Uneven floor surface observed around a drain cover in classroom D (for 19-47 months old).-Please fix and level the indicated floor area to reduce/prevent tripping hazards.

      • Hot water temperature was measured between 20 and 22°C in most washrooms.-Please increase and maintain hot water temperature between 38–43°C to ensure proper handwashing process.
