- Facility did not have a sanitizer for equipment and surfaces, which increases risk of cross-contamination. Corrective action: Please obtain one of the following approved sanitizers and its corresponding test strips to verify solution concentration1. Chlorine / bleach solution2. Quaternary ammonia solution (quats)3. Iodine solutionIf you would like to use an alternative sanitizer, please reach out to PHI for approval.

- Facility had access to a handwashing sink at the event, however did not have hand sanitizer at their booth. Hand sanitizer was obtained during the inspection. Going forward, please ensure that you have hand sanitizer at your booth for special events.
