      • There was a hole in the weather stripping at the back entry door.Replace the weather stripping or otherwise ensure the hole is covered to prevent pest entry.

      • A light cover was missing in the Deli area by the hand sink.- Install a new light cover.

      • A meat slicer was observed in the Deli area had food debris.- Have the Deli Slice disassemble, clean and sanitize every 2 hours.

      • Some clear plastic bowls were observed, stored in bulk containers, including flour and sugar, and icing powder, contact with the dry ingredients.**Please ensure that scoops for bulk dry ingredients are stored in one of the following ways:1) Outside the bulk container in a separate container.2) Inside the container with the handle facing up and not touching the ingredients.3) Use a clean and appropriate scoop with handles each time.

      • Mouse droppings were observed behind the large oven in the southeast of the building.Mouse droppings were observed in the base of the small white oven. - Have the area cleaned and sanitized.

      • A small deep fryer was observed in the Deli side of the store.- Remove the deep fryer as it is a restricted condition on the Food Permit.

      • Damaged/missing floor tiles in the front bakery sales area.Replace missing tiles or otherwise repair floor to ensure that it is smooth, non-absorbent to moisture, easy to clean, and constructed of material intended for the purpose.

      • 12-Sept-20241) The floor tiles in front of the bakery area have gaps, exposing the wooden surface in the hallway leading to the back of the bakery prep area.2) There are broken or missing wall tiles at the front of the bakery, behind the bread slicer.3) The baseboards are missing at the front of the bakery, next to the bread slicer equipment.4) There were some damaged areas of wall.***As a result of the above issues, debris has built up in these areas. Please repair the damaged areas.       • The handwashing sink at the back of the bakery has a hole in it and needs to be replaced.*All handwashing stations in the facility must be equipped with soap, single-use paper towels, and both hot and cold water at all times, and they must be in good repair.
    • 15. Is the facility free of a pest infestation?
      • 9 mice were observed in tin traps in the facility (one alive, 8 dead). An abundance of rodent droppings were observed on floors in the facility, as well as under the handwashing sink in the deli, and in the base of display coolers in the deli.Ensure that adequate pest control is taking place in the facility.

      • 1) Mice traps were observed on-site, but no pest control records were maintained. The operator mentioned they have a professional company responsible for setting the traps and conducting inspections once a month.*Please maintain pest control records on-site at all times.2) Flies were observed in the back of the bakery and deli prep areas.*Please keep these areas clean and free from pests, such as flies.

There were dry water marks visible on the plastic cover, but no water dripping was observed during the inspection.6) There were some damaged areas of wall.***As a result of the above issues, debris has built up in these areas. Please repair the damaged areas.       • 1) Bulk food in the prep area, including but not limited to dough, sugar glaze, breadcrumbs, and flour, was uncovered.2) Breadcrumbs were stored in milk crates lined with clear plastic bags.*The operator covered these items with lids during the inspection. All food (raw and/or cooked) must be covered at all times when not in use to prevent contamination.

      • - Only one drain plug was observed for the two-compartment sink in the back of the deli area.*Each compartment of the sink requires its own drain plug to properly wash, rinse, and sanitize dishes. Please correct this issue.* Reviewed the dishwashing procedure with operator and provided the two-compartment sink AHS sticker to follow.
