    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Could not observe client records at the time of the inspection including sterility documentation.Corrective Action:- Operator mentioned she keeps these records, but they are brought home.- Requested operator to provide a copy of a client's records for review.

    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Single-use disposable gloves were being re-used.Corrective Action:Ensure single-use disposable gloves are not re-used.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Disposable hair processing caps were are reused between clients.Corrective Action: - Discontinue the reuse of disposable hair processing caps. - Reusable processing caps (i.e. more durable vinyl/silicone caps) may be reused only if they are designed for reuse and can be cleaned and disinfected.
    • 20. Is reusable equipment disinfected properly?
      • Barbicide is not prepared as per manufacturer's instructions.Corrective Action:Discussed proper use of Barbicide as per manufacturer's instructions (this includes proper mixing of 62.5 mL with 1L of water and ensuring a contact time of 10 minutes).
