    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): The dining and lounge areas within the facility were noted to have some visible damage to the walls. Damaged walls within the community care facility could be seen to diminish the sense of dignity to persons in care. Submit by March 20, 2026, a written plan outlining your plan to address the walls in lounge and dining areas. The plan must include the system you will implement to support ongoing monitoring of the physical facility.
      • R7.1I - Maintain all rooms and common areas in a good state of repair; 22( 1 )(b)
    • R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
      • Observation(s): Of the records reviewed, two persons in care did not have oral care plans. Staff confirmed that all persons in care are assisted/supported with oral care regularly. Lack of information guiding staff as to what oral care interventions are needed could lead to inconstant oral care provision for persons in care. Submit by March 20, 2026, a written plan outlining how you will ensure all persons in care have a oral health care plan. The plan must include the system you will implement to support ongoing monitoring to ensure sustained compliance.

    • R3.1 - Do the manager and employees meet the employment requirements and are they assessed regularly for performance?
      • Observation(s): The system to ensure that all employees comply with the policies and procedures of the medication safety advisory committee is ineffective. A review of medication administration records identified several instances where PRN (as needed) medication effectiveness was not documented. *This is an ongoing contravention.
      • R3.1AA - Ensure that all employees comply with the policies and procedures of the medication safety and advisory committee; 68 ( 4 )
    • R4.6 - Are facility records current and complete?
      • Observation(s): The system to ensure that a record of menu substitutions is kept is ineffective. Facility staff reported that no record of menu substitutions is kept.
      • R4.6C - Retain food services records of menus and menu substitutions; 87(b)
    • R5.1 - Does the facility provide food services which meet nutritional needs and preferences for persons in care?
      • Observation(s): The system to ensure that a menu is developed for at least a four week period is ineffective. Facility staff reported that a four week menu was not developed.
      • R5.1B - Develop a menu for persons in care accommodated for greater than 6 weeks, for at least each 4 week period; 62( 1 )(b)

    • R7.2 - Is the environment maintained to prevent falls?
      • Observation(s): During the inspection it was noted that one person in care has laundry facilities located in his bedroom, which did not have slip resistant surface. It was noted that the laundry facilities located on the main floor of the home did have slip resistant floor surface.
      • R7.2R - Laundry facilities must have a slip resistant floor surface if used by persons in care; 35( 2 )(a)
    • R3.1 - Do the manager and employees meet the employment requirements and are they assessed regularly for performance?
      • Observation(s): As needed medication (PRNs) effectiveness was noted to be inconsistently documented.
      • R3.1X - Ensure that all employees comply with the policies and procedures of the medication safety and advisory committee; 68 ( 4 )
    • R6.3 - Does the facility demonstrate appropriate outbreak prevention and control measures?
      • Observation(s): It was identified during the inspection that there is currently no system in place to monitor fridge and freezer temperatures to ensure safe storage of food.
      • R6.3B - Ensure that food is safely prepared, stored, served and handled; 63 ( 1 )
    • R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
      • Observation(s): It was identified during the inspection that medication care plans did not indicate review or revisions dates upon completion. This infraction was corrected during the inspection, and the manager is developing a system to ensure ongoing compliance.
      • R10.3J - Each care plan must be monitored on a regular basis to ensure proper implementation; 81( 4 )(a)

      • Observation(s): The system for ensuring that all rooms are maintained appeared ineffective. The living room wall shows repeated damage in one area. Please notify licensing by the above date of the plan you develop to address this.

    • R2.1 - Are written policies and procedures in place to guide employees in all matters relating to the care and supervision of persons in care?
      • Observation(s): The system in place for ensuring policies and procedures are reviewed/revised at minimum annually does not seem to be working. The policy and procedure manual at the house indicated that the review of polices should have occurred in November of 2016.
      • R2.1Q - Review and, if necessary, revise policies and procedures at least once a year; 85( 1 )(b)
    • R10.2 - Do care and supervision practices ensure health, safety and dignity of persons in care?
      • Observation(s): The process of ensuring all residents at risk of elopement are being fitted with ID which cannot be easily removed is not occurring. One resident had a Velcro shoe tag in place. Another resident who is screened as requiring this type of ID has removed his shoe tag and iron-on patches.
      • R10.2M - Ensure that persons in care who may leave the facility without notifying an employee and may not be capable of identify themselves be fitted with a bracelet or other means that cannot be easily removed, indicating the person's name, facility, and emergency contact information; 56( 3 )(a)(b) (Show More)

    • 5.1 Does the facility provide food services which meet nutritional needs and preferences for persons in care?
      • Observation(s): Although there is a menu in place, it is not being consistently completed to indicate what was served to persons in care.
      • Provide for each day, a nutritious morning, noon and evening meal, with each meal containing at least 3 food groups described in Canada's Food Guide; 62( 2 )(a)
    • 7.1 Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): Since the last inspection, the banister has been re-painted. However, it is showing signs of wear again and paint is peeling off in some areas.
    • 9.1 Are medications stored, handled, and administered appropriately?
      • Observation(s): Although there was evidence of a medication room inspection, there was not any record of the medication safety advisory committee meeting.
      • Appoint a medication safety and advisory committee consisting of the manager or person designated by the manager, the supervising pharmacist and, if employed by the licensee, the health care provider responsible for the immediate supervision of health care services provided in the facility; 68( 1 )(a)(b)(c) (Show More)

      • Observation(s): One resident's elopement care plan was outdated at the time of the inspection. This was promptly reviewed and corrected during the inspection.
      • Review and, if necessary, modify each care plan if there is a substantial change in the circumstances of the person in care, or at least once a year, to ensure it continues to meet the needs, preferences, and is compatible with the abilities of the person in care; 81( 4 )(b)(i)(ii)
