      • The pedicure basin had a build-up of debris.Clean and disinfect equipment properly after each client.
    • 20. Is reusable equipment disinfected properly?
      • CS 20 disinfectant had expired in November 2024.The solution was discarded during the inspection. An unexpired CS 20 was available on-site.

      • 1) A low-level disinfectant was not available for reusable hairstyling equipment such as hair combs and brushes.Provide a low-level disinfectant such as a barbicide.2) The operator stated that reusable equipment for nails was disinfected in the CS 20 solution for 15 to 20minutes and the disinfectant was replaced when the solution changed color or appeared unclean.Follow the manufacturer's instructions for use including contact time and reuse period.

      • There was no proper ventilation for performing acrylic nail services.Cease acrylic nail services and remove all advertisements for this service if unable to meet ventilation requirements.

      • The fabric client chairs were used for pedicure, hair styling and temporary makeup. The operator did not have means to clean the chairs after each client.Replace chairs. Work and client surfaces must be smooth, non-porous and easy to clean.

      • Operator had cosmetic products which did not have a full or legible ingredients list:- Clear vial (reported to contain acetone).- Sky Zone Glue DebonderOperator has stated these products will not be used on clients.

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • The following written procedures were not developed at the time of the inspection:- the cleansing of skin and mucous membranes.- post-service care for personal services that involve energy emitting equipment.REQUIREMENT: Complete the above written procedures and send to the PHI for review.

      • Cleanser was not available to cleanse the skin prior to the waxing and facial services.REQUIREMENT: Obtain an appropriate cleanser.
    • 11. Is post-service care provided by personal service workers following any personal service or accident where a client's skin or mucous membrane is cut or punctured during a personal service, or when energy-emitting equipment is used?
      • No post-service care instructions were available for clients after energy emitting services are conducted.REQUIREMENT: Provide both verbal and written instructions to clients after providing laser service. Send a copy of the written post care for review.

      • Operator has CS20 disinfectant as well as appropriate testing strips, but had not developed a high-level disinfecting log.A template log was shown to the operator at the time of the inspection.REQUIREMENT: Complete the above sections of the template log.

      • No medical history form was developed to screen clients for contraindications, or other skin conditions prior to beginning the laser services.REQUIREMENT: Develop a medical history form and ensure it is compliant with any manufacturer instructions.
