- *Multiple food equipment, utensils, and storage areas throughout the serveries require increase cleaning and sanitizing- heavily stained coffee cups were found in the cupboards and drawers in multiple serveries-dirty soup bowls were found in the memory care units -food splashes and food debris noted inside microwaves in multiple serveries-debris and pink slim noted in the ice machine tray and on dispensers-breadcrumbs noted around the toaster area on the 4th floor servery-debris noted in multiple drawers in multiple serveries-sticky residue noted multiple fridges in multiple serveries

Ensure all parts of the tables are included in routine cleaning and sanitizing.*Black dust/debris noted on the ceiling areas right next to the ventilation vents. Please clean area and ensure ventilation is on good working condition.

- *Unwrapped tea bags were stored in open plastic containers with holes in all serveries. Change storage practice. Ensure tea bags are protected contamination (cleanable container with lid, handled with a utensil) or serve individually wrapped tea bags. Tea bags were discarded on site.

- *Paper towel dispenser was empty at hand sink in the north servery. Restock dispenser and ensure hand washing supplies are available at all times.

Paper place mats were no longer in good condition. Replace paper mats with cleanable liner.

- Food for trays observed stored at room temperature. Reviewed ensuring that trays are dispensed as delivered and not dispensed extended periods of time prior to service. Keep items in steam table. Ensure that food items are stored directly in steam table and cold items are stored in a manner that keeps them cold or stored in fridge until service (eg. dessert).

- Freezer in North Memory Care unit verified at 8 C, food products inside were melted. Ice cream, ice cream sandwiches, and frozen pretzels discarded during inspection. Ensure that cold storage units are able to maintain temperature. Repair, replace, or remove freezer.
