    • 03. Are clients at this facility being protected from hazards (Critical)?
      • There were chemicals present in the upstairs bathroom cupboards. There was a Swiffer sweeper present within the main floor bathroom. There were no locks/methods present to prevent access to the chemicals by the children.*This has been noted in the past
    • 15. Is laundry being properly handled in a suitable manner?
      • There were ~4-5 snowsuits stored along a table in the infant's room.The operator indicated these were to be laundered.Ensure soiled laundry is never stored on "clean" surfaces. Soiled laundry should be stored in a method that prevents contamination of laundry or other surfaces.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • There was a bucket of water present in the main floor bathroom.The operator indicated that the purpose of the bucket was to be used for soaking soiled underwear before being placed in the laundry machines.The bucket was open and accessible to the children. If this practice is to be used (strongly recommend this practice not be used), the bucket of water cannot be accessible to the children.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • i. The sanitizer solutions on-site measured 0 ppm at the time of inspection.There was a bottle of sanitizer labeled as 100 ppm bleach/water. The chlorine/water solution inside measured 0 ppm.ii. There was also a bottle of disinfectant on-site labeled as 1000 ppm bleach/water. The chlorine/water solution inside measured 0 ppm.The bulk bleach solutions present on-site measured ~200 ppm at the time of inspection.iii. The operator was unable to locate chlorine test strips to verify the concentration of the bleach/water sanitizer solutions.

      • i. The gates at the entrance to the infant's room and the stairwell to the basement were made of unfinished wood.ii. The gate to the basement was pulling away from the wall and not properly secured.

      • i. The surface along the top of the white table in the Toddler's room was worn. As a result, it was no longer smooth and impervious to moisture.ii. The inside of the toy bins in the Toddler's room were soiled. Please increase the frequency of cleaning for these spaces.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • The operator's personal items were being stored in contact with the daycare items.This included:i. The operator's personal hairbrushes/blow dryer being stored in the upstairs OSC room drawers.ii. The operator's weight scale was present in the upstairs OSC bathroom.iii. The items in the closet in the OSC portion of the home were not stored in an organized manner. The items were all mixed together.iv. The laundry adjacent to the kitchen was not stored in a tidy manner. There were personal sets of beds/sheets (stored within bags) mixed in with clean, extra sets of sheets/bedding.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • -The grate over the HVAC vent in the Baby Room was broken in multiple areas. This could lead to children getting caught on the sharp edges.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • -There was Baby Powder observed in the diapering area. This can cause breathing problems and cannot be used in child care facilities. Please remove.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • -There were expired first aid supplies. Ensure supplies are being checked monthly and replaced as needed.
    • 15. Is laundry being properly handled in a suitable manner?
      • -Clean and soiled laundry items are not being properly managed in this social care facility. There were no laundry bins. Dirty laundry was being stored on the floor in the diapering area.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • -Surfaces and equipment in this social care facility are not being cleaned and disinfected properly and/or at suitable intervals. There was a badly stained plush toy observed in the Baby Room. It was removed at the time of inspection.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • -Toilet paper in washrooms not stored on the holder. Ensure toilet paper is stored on the holder.
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • -A worker at this social care facility is not washing hands frequently and/or in a proper manner. Observed a staff member wash their hands at the kitchen sink and then wipe the excess water on their shirt instead of using a single use towel.

      • -The surface of the ceilings, walls, and/or floors in this social care facility are not acceptable. There was a hole in the wall behind the entrance door, please repair.

    • 05. Are communicable diseases at this facility being properly managed?
      • -There was a young child with what appeared to be red rashes on her arms and several red bumps on her face. The operator was not aware of this when it was first brought up to her and later said they were mosquito bites. Parents should be taking this child to the doctor and providing a note as it is unclear what the marks are from and could be something contagious-Illness logs had not been filled out since November 2022; ensure they are continually being updated.-Illness logs on August 10, 2023 were still not updated
    • 06. Are proper protocols being followed in regards to client care?
      • -The downstairs diapering area was cluttered. Please remove the two extra diaper pads, jackets and other items stored in the diapering area.-The upstairs washroom had a diaper pad but this space has not been approved for diapering. Please remove the diaper pad.
    • 08. Is this facility properly constructed (Non-Critical)?
      • -The surface of the ceilings, walls, and/or floors in this social care facility are not acceptable. There was a hole in the wall behind the entrance door, please repair.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • -The first aid kit for this social care facility is not being properly managed, is not readily available, and/or is not properly supplied. Some supplies had expired. Please ensure kits are updated.
    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • -The fire alarm had been taken down in the upstairs overflow room. Please ensure the fire alarm is installed.

      • -Chemicals in the upstairs washroom were not secured and the storage closet at the back of the washroom was also left open and accessible to children. Ensure chemicals are secured and the closet door is kept closed.
    • 05. Are communicable diseases at this facility being properly managed?

      • -The handwashing basin in the upstairs washroom was not equipped with adequate supplies. Ensure soap and single use towels are available.

      • -The first aid kit for this social care facility is not being properly managed, is not readily available, and/or is not properly supplied. Some supplies had expired. Please ensure kits are updated.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • -Surfaces and equipment in this social care facility are not being cleaned and disinfected properly and/or at suitable intervals. Chlorine was measured at 25ppm. Ensure a 100ppm solution is used. A new solution was prepared at the time of inspection.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • -One of the sinks in the upstairs washroom was dripping. Please repair.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • -Storage facilities for this social care facility are not providing adequate separation of stored items. Children’s personal items were stored on chairs at the front entrance. The operator moved them to their cubbies at the time of inspection.
