      • When asked to test the disinfectant for reusable equipment, the operator displayed inadequate knowledge on how to use the test strips by submerging the test strips in the disinfectant for longer than required. **Follow all manufacturer's instructions when using test strips to ensure the accuracy of results.**The solution was re-tested according to manufacturer's instructions.

    • 17. Is reusable equipment cleaned properly (Critical)?
      • 1. There is no cleaning step for the reusable tweezers before disinfection. Tweezers are soaked into CS20 immediately after client use and then rinsed with water. Please obtain a cleaning tool and detergent to thoroughly clean the reusable tweezer, followed by a rinsing and drying step before disinfection.
    • 20. Is reusable equipment disinfected properly?
      • 1. CS20 is reused for the 14 days without testing its concentration. Please obtain appropriate test strip and test the disinfectant prior to use daily. A monitoring log was provided at time of inspection.2. The lash and glue rings were not cleaned; there were glue still stuck onto the rings. Unless there is validated instructions for cleaning and disinfecting by the manufacturer, these must be discarded after each use.3. Tweezers were not fully submerged into the CS20 for adequate disinfection. Operator was suggested to obtain a container that would minimize waste and allow the tweezers to be fully submerged in disinfectant each time.

      • Ingredient list was not available on the glue used for eye lash extension. Please obtain its complete list of ingredients and instruction for use.
