    • 03. Are clients at this facility being protected from hazards (Critical)?
      • -The cord for the Christmas tree in the Pre-school Room was very long and laying on the floor in the play area. This is a potential trip and strangulation hazard. Please ensure the cord is inaccessible to children.- There were missing electric outlet plug covers in the Pre-school Room. Please ensure covers are in place.-The cupboards above the diaper table in the Toddler Room are low and children could hit their head. Please install padding.

      • - The Chlorine spray bottle in the OSC was left on a low counter, accessible to children. The bottle was moved out of reach at the time of inspection.

      • -There was a purse with long straps in the play clothes section of the OSC. This is a potential strangulation hazard. Please remove.-There were three white cubbies in the OSC Room that were not secured to the wall. Please secure.-There was a loose screw on the OSC Room side of the cubbies used as a room divider. Please repair.

      • -There was a diaper mat stored on the floor in the Baby Room and Toddler Room. Ensure the mat is stored off the floor and out of reach of children.-Please ensure diapering procedures are posted in the Toddler Room.-Diaper supplies were not stored separate in the Toddler Room. Please ensure each child’s diaper supplies are stored separate.

      • -The sealant behind the sink in the Baby Room had black residue. Please redo the sealant.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • - The Chlorine spray bottle at the toy washing sink was not labelled and the solution was at >1000ppm
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • * Cleaning was noted as a concern in the following areas:-The lego table in the OSC and Baby Rooms-Toy storage bins in the Pre-school Room-The tape on the area rugs in the Toddler and Pre-school Rooms was peeling and collecting debris. Please remove tape and ensure the floor is left in a smooth and easy to clean condition-The floor in the Toddler Room was left sticky from tape. Please ensure all tape residue is removed.-Tape was peeling off the kitchen set toy in the Toddler Room. This prevents the surface from being easy to clean. Please ensure it is clean and in a condition that is easy to clean.-The fridge in the Baby Room had a large build-up of ice. Please defrost and clean.- Some of the larger toys in the Baby Room were visibly unclean. Please clean.-The wall in the washroom next to the handsinks was visibly unclean on the wall and floor.

      • -There was a sensory bin in the Pre-school Room with no lid and rice left inside. Please ensure that food used for sensory is always kept covered and is replaced frequently.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • -Sleeping mats were stored in dividers with sheets on and the sheets were all touching. Please ensure that sheets are stored separate from mats and mats are cleaned and disinfected after each use.

      • - Diaper tables were being used to store crafts, ipad and a bench. Ensure diaper tables are not used for storage.- The diapering sink in the Toddler Room was being used to rinse paint brushes. Ensure the diapering sinks are used for handwashing only
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • -The water table in the Toddler Room was unstable. Please repair.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • -The kitchen door was left open and cupboards used to store chemicals were not secured. Please ensure the kitchen door remains locked at all times and chemical cupboards are kept locked.-Bleach spray bottles throughout the facility were not appropriately labelled. Please label.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • -There was an area in the Pre-school Room that was used for storage. Please ensure this area is inaccessible to children.-There was a shelf in the Infant Room and another in the OSC Room that were over 4 feet. Please secure the shelves to the wall.
    • 06. Are proper protocols being followed in regards to client care?
      • -Diapering in the Infant and Toddler Room was being done on a floor mat. Please install diaper change tables in these rooms.*Ensure diapering procedures are posted
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • -There was peeling tape on the Kitchen toy in the Infant Room. Please remove or replace the tape.-There were foam blocks in the OSC and Pre-school Room that were visibly unclean and had bite marks. Please remove unclean and damaged foam blocks.-The lego table in the OSC Room was visibly unclean. Please clean.

      • -There was a slide in the Infant Room kept on carpet. Please install protective surfacing or remove the slide.
