      • A bed and bedding items were observed in the back area of the facility.The operator was informed that he or any staff member cannot sleep in the facility. The operator confirmed that no one will sleep in the facility.

      • Used single-use roll on wax cartridges were observed in the wax warmer machine.The operator discarded the used roll on wax cartridge on PHI's request and was advised not to re-use them.

      • Feb 19: -Single use implements were not being used to dispense product; wooden spatulas were being stored inside the product. Ensure that wooden spatulas are not stored inside product and only single-use implements are used to dispense product.Please ensure that measures are put in place so that pedicure scrubs and creams are dispensed without contaminating the bulk supply.Previous inspection-Single use implements were not being used to dispense product; wooden spatulas were being stored inside the product. Ensure that wooden spatulas are not stored inside product and only single-use implements are used to dispense product.- Steps are not being taken to protect the contamination of the bulk supply of pedicure scrubs and creams.       • - Porous or uncleanable equipment (wooden spatula to scoop out pedicure scrubs and creams) is not being discarded after each use. The operator was asked to discard all the used wooden spatulas after single use. Equipment classified as porous or uncleanable must be discarded after use in one personal service on one client.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • - Used semi-critical equipment (callus remover) was not being adequately cleaned prior to disinfection. A soiled semi-critical equipment was observed along with cleaned equipment.The operator was advised to assess the reusable equipment after cleaning them. The operator kept the callus remover in the reprocessing for cleaning.Ensure equipment is: disassembled, cleaned to remove soil, and rinsed and dried prior to disinfection.
    • 20. Is reusable equipment disinfected properly?
      • - Equipment (Foot filer/Callus remover) is not being fully immersed in the high-level disinfectant during the disinfection process. The container in which the semi-critical tools are immersed for disinfection is not big enough to accommodate the callus remover for adequate disinfection. The operator was advised to acquire an appropriate container with a lid.Ensure equipment is fully immersed in solution.- Disinfection records were not available. Ensure records are kept of: test dates, whether the solution passed/failed, when the disinfectant was changed and the worker’s initials who performed the activity.
