      • 1. Cooked food was noted being reheated in the steam table. - Ensure cooked food is reheated on the stove or in other manners that can bring the food temperature to 74 quicky. 2. Frozen chicken was noted being thawed at room temperature. - Ensure frozen food is thawed in proper manners, such as in a cooler.

      • One live cockroach was found under the dish washer. (Pest control devices were present through out the facility. Pest control technician serviced the site and provided a service report on the day right after the last inspection. The service report was provided to inspector.)
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • 1. The wall behind the dishwashing machine is in disrepair. - Ensure the holes and gaps around the pipes are properly sealed. 2. There are cracked floor tiles in the kitchen. - Ensure the cracked floor tiles are replaced.

      • 1. Two live cockroaches were found on the floor in the dishwashing area. 2. Mouse drippings were found on the floor and a shelf in the dry storage. ( Mouse traps are present in the area. )- Ensure a licensed pest controller is retained to eliminate the pests from the facility.

      • 1. The wall behind the dishwashing machine is in disrepair. - Ensure the holes and gaps around the pipes are properly sealed. 2. There are cracked floor tiles in the kitchen. - Ensure the cracked floor tiles are replaced.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • Floors under the dishwashing machine, sinks and in the hard-to-reach corners require cleaning. All cardboards under the sink are to be removed.
