A peeling toilet seat cover was observed in the Preschool C washroom.-Please replace the toilet seat cover to ensure it can be properly cleaned.3. Protruding screws were also observed in the Preschool C washroom.-Please install appropriate covers for the screws attached to the toilet.

Push pins were observed on walls of some of the classrooms.-Please remove the push pins as these may fall onto the floor and injure a child. Use tape or sticky tacks instead.

      • 1. Materials stored around the infant diapering area.-Please remove all materials around the indicated area to prevent contamination.2. Staff cleaning children's toys in the handwashing sink adjacent to the infant room diapering area.-Please do not clean toys in sinks adjacent to the diapering area. Toys must be cleaned and sanitized in a designated sink to prevent contamination and the spread of infection.3. Dirty mat with rough surfaces observed in the infant diapering area.-Please replace the indicated mat with a smooth, non-absorbent, and easy-to-clean diapering mat.

Sensory bottles with multiple layers of adhesive tapes were observed Infant A classroom-Please seal sensory bottles with non-toxic glue.Openable sensory bottles may expose children to potentially hazardous substances, and the use of multiple adhesive tapes makes the bottles difficult to clean and sanitize.2. 2. Cane rattan toy backet which could be difficult to properly clean and sanitize, was observed in preschool 1B room.-Please replace such toy containers with smooth, nonabsorbent and easy-to-clean containers.

Staff stated that request has been initiated to re-level the concrete.*Ensure this is corrected to enable access to the door during emergency for safety of the children and staff.2. The classroom main door not in good working condition due to the malfunctioning door latch and the door lock was held with duct tape.*Repair/replace
