    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No written policies for skin cleansing, antiseptic use prior to services and accidental cuts were available.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility assurance documentation for sterile equipment was not retained with the clients' records. Ensure that sterility assurance documentation for all critical equipment used during services is retained with clients' forms.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Wooden foot files were being re-used. Singel-use tools must be discarded after the service on one client.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Some cosmetic products did not have appropriate labels (unlabelled bottles were observed, and Cool Sculpting products did not have ingredients listed). Ensure that all cosmetic products have appropriate labels. Updated on June 12, 2026:Received emailed pictures of labelled bottles.

      • No written policies for skin cleansing, antiseptic use prior to services and accidental cuts were available.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • Unacceptable antiseptic product was used for skin preparation: alcohol swabs expired in November 2023. They were discarded during the inspection. Ensure that appropriate antiseptic products are used.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility assurance documentation for sterile equipment was not retained with the clients' records. Ensure that sterility assurance documentation for all critical equipment used during services is retained with clients' forms.

      • 1. Preempt RTU wipes expired in February 2026, and Cavicide disinfectant expired on September 21, 2024. Acceptable Preempt RTU was available and will be used for surface disinfection.2. No original bottle of Preempt HLD5 disinfectant was available. The solution tested in the soaking disinfection tray was tested as acceptable. Obtain new high-level disinfectant.
