- During the inspection it was found that the facility did not have a pest control service or a pest control checklist. The owner was doing his own pest control checks, but there were no records of the pest control checklists being completed. There was no pest evidence in the facility. Please ensure that regular checks for pest activity are conducted in the facility.

- 1. During the inspection a whole raw chicken and tub of raw chicken juice was found above vegetables in the walk-in cooler. The owner moved the raw chicken to a different location in the walk-in cooler where it would not be above any produce or cooked foods. Please ensure that all food is protected from contamination at all times.2. During the inspection it was discovered that there was a handleless scoop and a scoop with its handle buried in food inside the bulk food bins.Please ensure that all food is protected from contamination at all times.

- During the inspection an unlabeled spray bottle was found in the facility. When the operator was asked what was in the spray bottle, he responded that it was water. The operator then labeled the unlabeled spray bottle.Please ensure that all spray bottles in the facility are labeled correctly.
- 11. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Critical)?
- During the inspection the chemical dishwasher was found to have no chlorine present at the plate level. The 3 sink method of Wash, Rinse, Sanitize was required to continue operations. A dishwasher technician will be there the next day to assess the issue. Please ensure that the dishwasher is fixed so that there is chlorine present at the dish level.
