    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No written procedures for preparation of skin, client care, or use of a dual purpose sink were observed during inspection. Written procedures are required to ensure all staff has knowledge of processes to prevent harm and injury to the client. Ensure that the following written procedures are available during re-inspection.1. Preparation of Skin2. Client Care3. Dual Purpose sink.
    • 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
      • Bottles used for cleaning surfaces before disinfecting were unlabeled. Informed by the operator that they do not know what the chemical is called. All bottles should be labeled, and manufacturer's instructions are required to be available to ensure safe and correct use of cleaning chemicals. Ensure that the bottles are labeled and the manufactures instructions are available.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Single use nail files were observed at the back reprocessing sink in the storage room. Single use tools cannot be adequately cleaned and disinfected between client use and are required to be discarded after use. Ensure that single use items are being discarded after use.
    • 16. Is reusable equipment handled and stored properly (Non-Critical)?
      • Single use cloth towels were observed to be reused for cleaning multiple pedicure chairs. Single use towels are required to be cleaned after every use to prevent microbial growth and cross-contamination between multiple surfaces. Ensure that the clothes are laundered after every use.
    • 20. Is reusable equipment disinfected properly?
      • No high-level disinfectant logbook was available during the inspection. Semi-critical reusable tools are required to be reprocessed in high-level disinfectant, and solution is required to be tested with high-level disinfectant test strips to ensure adequate disinfection. Ensure that a logbook for high-level disinfectant is available during reinspection.
    • 20. Is reusable equipment disinfected properly?
      • Reusable foot files were observed to be submerged in dirty disinfectant solution. All reusable tools are required to be adequately cleaned before submerged in disinfectant solution. Ensure that all reusable tools are properly cleaned before disinfecting.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • All empty nail stations were observed to have nail dust debris covering the table surface and the chairs. All personal service facilities are required to be kept in a visibly clean state, and all environmental surfaces are required to be cleaned and disinfected after each client use. Ensure that all stations are cleaned and disinfected between customer use.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Food debris was observed in the reprocessing sink in the back room of the facility. Sinks used for reprocessing are required to be separated from activities such as food preparation to prevent cross contamination of the tools used in personal services. Ensure that the sink is only used for reprocessing of equipment.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • A toilet was observed in the back room with no handwashing sink. Informed by the operator that the reprocessing sink is used to complete handwashing. All sinks used for reprocessing are required to be adequately disinfected if used for handwashing. No written procedure was observed for dual use of the reprocessing sink. Ensure that the sink is disinfected between use and a written procedure is available during reinspection.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • 1. An outlet on the wall behind the pedicure chairs was observed to not have a cover plate. All outlets should be covered to prevent injury and to allow for adequate cleaning. Ensure that the outlet has a cover plate. 2. The washing machine was observed to have a leak. Water creates conditions for bacterial growth. Ensure that the washing machine is repaired.

    • 15. Is reusable equipment handled and stored properly (Critical)?
      • 1. Observed contaminated semi critical equipment stored with clean equipment at reprocessing area. Please increase storage area in reprocessing area and store equipment with adequate separation.2. Please remove crock pot from reprocessing area.Storing contaminated equipment with clean equipment increases the risk of cross-contamination. Ensure there is enough separation space at reprocessing area; there needs to be a clear separation between clean and dirty equipment.
    • 20. Is reusable equipment disinfected properly?
      • 1. Please ensure pedicure basins are cleaned with soap and water first, then, apply intermediate level disinfectant. 2. Manufacturer's instructions were not followed while disinfecting the equipment. Proper disinfection of equipment cannot be achieved without following the manufacturer's instructions.3. As per manufacturer's instructions, test strips must be used to determine if disinfectant solution is acceptable to be used more than once. Please obtain test strips for testing disinfectant re-use period to verify its efficacy. Please record each time disinfectant solution is changed, and record the date of the test on the 7th day of its re-use period. Disinfectant must be changed every 14 days regardless of test result.4. Obtain a container that is large enough to submerge all re-usable instruments in disinfectant solution.

      • Please obtain test strips to determine if disinfectant can be re-used, as per manufacturer's instructions. Obtain a container that is large enough to submerge all re-usable instruments in disinfectant solution.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The bed for esthetic procedures, such as waxing, needs to be replaced.
