- There was no paper towel available at hand sink. Ensure soap and paper towels are available.

- 1. CS20 was expired. Please obtain new bottle of CS20. 2. Operator was using alcohol to disinfect surfaces. Alcohol is not an appropriate disinfectant. Use low level disinfectant with DIN.3. Operator was not soaking tools in CS20 for 20 minutes. Ensure tools are soaked for a minimum of 20 minutes.
