- Sanitizer solution bottles (QUATS) were tested and verified at concentration of 100 ppm. Diversey D10 Concentrated Detergent Sanitizer (DIN 02238968) at the chemical auto dispenser located by the dishwashing sink was noted to have expired on 03/23/2025. The refills for the sanitizer in the back room were also noted to have expired on 12/14/2025. Operator was able to purchase bleach (chlorine) during the inspection. Public Health Inspector provided education on how to prepare a fresh bleach sanitizer solution, which was prepared at a concentration of 100 ppm during the inspection. Public Health Inspector left a few chlorine test strips with operator. Ensure that the chosen food grade sanitizer used at this facility is not expired and maintained at their required concentrations (100 ppm - Chlorine, 200 ppm - QUATS) for effective sanitization of food equipment, surfaces and utensils.

- A container of lettuce in the walk-in cooler was observed to be uncovered in the walk-in cooler. Operator covered the container with saran wrap during the inspection.Ensure that food items in containers are covered with an appropriate lid to prevent the introduction of contaminants.

- Adequate pest control monitoring is not being completed monthly. AHS pest control checklist template was provided to environmental services department (housekeeping) during the inspection. Environmental services stated they would complete the checklist monthly as required for the kitchen. Ensure that pest control records are completed monthly or on a routine schedule and available for verification/review when requested by a Public Health Inspector.**Note: No evidence of pests or pest activity was observed at the time of inspection.

- The refrigerated display case at the front service counter of the kitchen is measured at a temperature of 11 degrees C. This was verified by the thermometer in the display case, Public Health Inspector's infrared thermometer and probe thermometer. Operator was instructed to move all high-risk food items into the walk-in cooler. Operator stated they would put an out of service sign on the display case. THE ABOVE NOTED REFRIGERATION UNIT MUST NOT BE USED FOR FOOD STORAGE UNTIL IT IS SERVICED. Investigate the issue and ensure the affected refrigerated display case serviced. Refrigeration units must be capable of maintaining a temperature of 4 degrees C or lower for safe food storage.

Even when pests are not evident, a pest monitoring program must be in place. Please ensure to complete pest control records monthly and keep them on site for verification by a Public Health Inspector. **Note: There were no signs pest activity observed during the inspection

- A can of tomato soup in the dry food storage room was observed with a severe dent at the seam. Item was discarded by staff during the inspection. Food products that appear to be damaged in any way shall not be available for consumption. Ensure that canned food items are free of damage including, but not limited to, severe dents or bulging.

- A tray of eggs was observed to be stored at room temperature above the grill. PHI instructed staff to move the eggs back into the walk-in cooler. Raw whole shelled eggs must be maintained at a temperature of 7 degrees C during storage, display, packaging or transportation to prevent bacterial growth.

Ensure that utensils used for bulk food products have handles of sufficient length to prevent the part of the utensil that is handled by staff from coming in contact with the foods.
