(a) the applicable fee prescribed in Section 5;

(b) adequately designed and appropriately located hand-washing facilities;
- Handwashing station(s) must be supplied with paper towel and liquid soap at all times.

- In accordance with Section 81 of the Health Protection Act, you must: renew the permit.
