1024 Laurier Ave Kelowna BC V1Y 6B1 · Residential Care - Licensing

Inspection summary

Youth Recovery House has been inspected 4 times by Interior Health Authority since March 8, 2022. The most recent inspection, on November 6, 2025, recorded 4 infractions.

None of the 4 inspections on record were free of infractions. In the past 12 months there was 1 inspection with 4 infractions recorded. All recorded findings relate to upkeep, equipment or paperwork rather than direct health risks.

The number of infractions per inspection has stayed roughly the same over time.

What these findings mean

Health authorities word their findings differently, so we group them by what they mean for the people in care. Higher-risk findings are the ones that could directly make someone sick, and they matter more here because young children, older adults and people who are unwell are more vulnerable to infection.

  • Permits, records and postingLower risk

Operating permits, required signage, written procedures and logs. These findings are administrative: the documentation was missing or out of date at the time of inspection.

Recorded 8 times across 4 separate inspections, most recently on November 6, 2025. For example: 1 - Does the Licensee operate in accordance with their license and inform Licensing of any significant changes?

  • Equipment and utensilsLower risk

Equipment has to be made of cleanable materials, kept in good repair and stored properly. These findings are mostly about upkeep, like a cracked surface or worn part. They are generally lower risk but show how well the place is maintained.

Recorded 2 times across 2 separate inspections, most recently on July 5, 2024. For example: 1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?

  • Other findingsLower risk

Findings that do not fit the categories above, such as follow-ups that confirm an earlier violation was corrected, or requirements specific to this type of facility.

Recorded 6 times across 4 separate inspections, most recently on November 6, 2025. For example: 3 - Does the facility demonstrate appropriate outbreak prevention and control measures?

4 inspections

  1. Routine Inspection

    4 infractions

    • R1.1 - Does the Licensee operate in accordance with their license and inform Licensing of any significant changes?
      • Observation(s): Note: The Residential Care Regulation reads: 46(2)(c) a licensee must not accommodate persons less than 19 years of age if persons aged 19 or older are also accommodated in the community care facility. During discussions with the manager, it was discovered that persons in care do not always leave the facility when they turn 19 years of age. Exemption requests are not being submitted when this occurs. When adults and youth reside in the same facility the risk to both age groups may be increased if there is a power imbalance due to age or progression in the program. Risk may also arise if there are differences in cognitive and emotional development between these age groups. When this was discovered, an immediate health and safety plan was requested by Licensing. The Manager submitted the plan as requested and it was accepted by Licensing. Submit to Licensing by November 26, 2025, a plan which details how persons in care over the age of 19 will not be in care with persons under 19 years of age. The plan must include a system for ongoing monitoring to ensure sustained compliance with the legislative requirements.
      • R1.1V - Exclude care to persons less than 19 years of age in an adult facility; 46( 2 )(c)
    • R2.1 - Are written policies and procedures in place to guide employees in all matters relating to the care and supervision of persons in care?
      • Observation(s): Through discussions with the manager, it was determined that there is no system to ensure policies and procedures are reviewed at least once a year. The manager advised that the policies typically get reviewed throughout the year; however, there is nothing to document or track the review to ensure that all policies and procedures are being reviewed as required. Polices and procedures may provide insufficient guidance for staff if they are not reviewed regularly to ensure all information remains up to date. This may increase risk to persons in care if policies and procedures have been updated but not reflected in the documents. Submit to Licensing by November 26, 2025, a plan which details how policies and procedures will be reviewed and, if necessary, revised as per the legislation. The plan must include a system for ongoing monitoring to ensure sustained compliance with the legislative requirements.
      • R2.1Q - Review and, if necessary, revise policies and procedures at least once a year; 85( 1 )(b)
    • R4.1 - Are person in care records current, complete and kept confidential?
      • Observation(s): Four persons in care's charts were reviewed and it was noted that in two of the charts, monthly weights had not been recorded for the months of July, August and October. When weights are not obtained and documented it may be more difficult to monitor and follow up on any unexplained weight changes. Submit to Licensing by November 24, 2025, a plan which details how monthly weights will be obtained and documented as per legislative requirements. The plan must include a system for ongoing monitoring to ensure sustained compliance with the legislative requirements.
      • R4.1U - Weigh each person in care monthly and record their weight in their nutritional plan (Does not apply to Hospice); 83( 4 )(a)(c)
    • R6.3 - Does the facility demonstrate appropriate outbreak prevention and control measures?
      • Observation(s): It was noted that a refrigerator used for laboratory specimen containers also contained beverages for persons in care. Storing these items together may increase risk of cross contamination and illness. Submit to Licensing by November 26, 2025, a plan which details how food items will be safely stored. The plan must include a system for ongoing monitoring to ensure sustained compliance with the legislative requirements.
      • R6.3B - Ensure that food is safely prepared, stored, served and handled; 63 ( 1 )
  2. Routine Inspection

    4 infractions

    • R6.2 - Have all persons admitted to the facility complied with the Province’s immunization and tuberculosis control programs?
      • Observation(s): The Licensee's system to ensure tuberculosis tests are obtained as required was found to be ineffective. During the inspection, three persons in care charts were reviewed and in two charts the tuberculosis screening documents indicated that a tuberculosis test was required within 30 days. Despite a skin test being required, there was no evidence to indicate that this had been done within that time frame.
      • R6.2A - Ensure that all persons admitted comply with the Province’s immunization and tuberculosis control programs; 49 ( 1 )
    • R1.1 - Does the Licensee operate in accordance with their license and inform Licensing of any significant changes?
      • Observation(s): The Licensee's system to self-monitor was found to be ineffective. During the inspection, it was noted that there was no monitoring in place to ensure water temperature remains below 49 degrees Celsius as per legislative requirements. The system to ensure tuberculosis tests are obtained as required was also found to be ineffective.
      • R1.1W - Regularly monitor the physical environment and the care and services provided; 61
    • R2.1 - Are written policies and procedures in place to guide employees in all matters relating to the care and supervision of persons in care?
      • Observation(s): The system to ensure orientation checklists are completed as required was found to be ineffective. During the inspection, three employee files were reviewed and two were found to have incomplete orientation checklists. It should be mentioned that the incomplete checklists were for employees cross-trained to other facilities and the manager noted that the completed checklists were most likely at the other facilities; however, there is no system to ensure this information is transferred to all facilities.
      • R2.1S - Ensure policies are implemented by employees; 85( 1 )(d)
    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?

  3. Routine Inspection

    3 infractions

      • Observation(s): During the inspection it was reported that staff hired prior to June 2022 may not have the correct criminal record checks in place as required by the legislation. At that time an immediate health and safety plan addressing the non-compliance was requested. The Licensee provided a plan that was accepted.
      • R3.1B - Ensure criminal record checks are obtained for all employed persons; 37( 1 )(a)
    • R4.6 - Are facility records current and complete?

    • R5.1 - Does the facility provide food services which meet nutritional needs and preferences for persons in care?
      • Observation(s): The Licensee does not have a system in place to ensure that the menu includes 2 nutritious snacks per day. During the inspection it was noted that the menu includes 3 meals but does not include snacks.
